Objectives:The Moxifloxacin, Levofloxacin and Amoxicillin/ clavulanate potassium were used to treatment AECOPD, We wanted to choose a better empirical antibiotics by observation the clinical efficacy of 3 kinds of drugs for treatment acute on chronic obstructive pulmonary disease exacerbation.Methods: A total of 90 patients were enrolled this study in Department of respiration in the Central Hospital hospital from October 2013 to February 2015, The patients were randomly divided into three groups, moxifloxacin group, levofloxacin group, amoxicillin / clavulanate potassium group, each group had 30 patients. Moxifloxacin group: moxifloxacin injection intravenous 400 mg, maintain 90 min infusion time, 1 times /d, the period of treatment were 7d; Levofloxacin group: Levofloxacin and Sodium Chloride Injection 400 mg intravenous drip, 1 times /d, maintain 90 min infusion treatment time, the period of treatment were 7d; Amoxicillin / clavulanate potassium group: in amoxicillin sodium and clavulanate potassium for injection of 1.2g dissolved in 100 ml Sodium Chloride Injection, 30 minute intravenous drip, 4 times /d, period of treatment is 7d. Three groups of patients had the treatment with oxygen inhalation, atomizing inhalation hormone, bronchiectasis and nutritional support and other symptomatic treatment at the same time. The changes were observed before and after treament in clinical symptoms and signs, blood routine test and hs-CRP value, pulmonary function, blood gas analysis and clinical efficiency, sputum bacterial clearance rate and adverse reaction.Results:The overall symptoms, patient baseline vital signs, lung function in patients, blood and hs-CRP and the blood gas analysis were better than before treament(P< 0.05). The FEV1, FEV1/FVC% value, hs-CRP value, blood routine examination and bacterial clearance rate of Moxifloxacin group were better than levofloxacin and amoxicillin / clavulanate potassium group,(P < 0.05), no statistical difference.were found between levofloxacin and amoxicillin / clavulanate potassium group. The total efficiency of three groups were 86.67%, 63.30% and 60.00%,respectively. The total efficiency of moxifloxacin were significantly higher than levofloxacin group and amoxicillin / clavulanate potassium group(χ2= 4.356,P=0.037;χ2= 5.455,P=0.020).Conclusions: The AECOPD patients treated by Moxifloxacin were safe and effective, moxifloxacin was safer, more efficient compared with levofloxacin and amoxicillin /clavulanate potassium in initial empirical treatment, it can be widely used, worth clinical promotion.
