Objects :1) To preliminarily analyze the high-risk HPV infection rate of the nasopharyngeal carcinoma(NPC) in NanChong. 2)To preliminary compare the short-term curative effects of locally advanced nasopharyngeal carcinoma(III-IVa Stage) with different HPV infection status. 3) To evaluate the values of different HPV infection status on predicting the curative effects with nuclear magnetic resonance(MRI) –DWI.Methods: 1) Collected the tumor tissue and extract its DNA from the newly diagnosed patients with nasopharyngeal carcinoma(NPC) in NanChong; determined the HPV-DNA with Fluorescent Quantitative Analysis-PCR. 2) Before treatment and one month after treatment, locally advanced nasopharyngeal carcinoma accepted MRI and nasopharyngoscope test and compared ⊿ADC values of different effect group. 3) Divided the patients with locally advanced nasopharyngeal carcinoma into two groups according to different HPV infection status; contrasted the short-time curative effects of the two groups referring to RECIST criteria. 4) Analyzed the ADC values of different HPV status and evaluated the effect predictive value of HPV infection status to locally advanced nasopharyngeal carcinoma.Results: 1) The HPV infection rate in Nan Chong was 30% for NPC patients and type-16 was the most common one with a rate of 20%. 2) The⊿ADC value of CR group were higher that of PR group,which was(0.976±0.122)*10-3s/mm2、(0.854±0.168)*10-3s/mm2(P<0.001), respectively. 3) The CRrate, PRrate and ORR rate of HPV-positive and HPV-negative groups were 60.0%,40.0%,100%; 62.1%,37.9%,100%,respectively(P>0.05). 4) Before treatment, the ADC values of HPV-positive and negative patients were(0.955 ±0.069)*10-3s/mm2 and(0.896±0.073)*10-3s/mm2, respectively, with a difference of statistically significance(P=0.011). After one-month treatment, the ADC values of HPV-positive were higher than negative group(P=0.012),which were(1.851±0.161)*10-3s/mm2,(1.700 ±0.196)*10-3s/mm2, respectively. The change values(⊿ADC) of before and after the treatment were not different between the HPV-positive group and HPV-nagative group,which were(0.80±0.03)*10-3s/mm2、(0.90±0.04)*10-3s/mm2, respectively(P>0.05). Conclusions: 1) There was a difference HPV infection rate of the NPC patients between Nan Chong and other regions except Tai Wan. 2) The region locally advanced nasopharyngeal carcinoma ADC values change a bigger group before and after treatment in the near future curative effect is better 3) The HPV infection status of the locally advanced NPC patients can not be the predictor of short-term curative effects in Nan Chong district. 4) Further researches need to be done about HPV infection statuspredicting curative effects with the MRI- DWI.
