The Etiological Classification Of Acute Ischemic Stroke And Analysis Of Medical Quality In Certain Mountain Area Hospital In Guangdong Province

ObjectsTo understand the etiological classification of acute ischemic stroke and themedical quality in certain mountain area in Guangdong province.MethodsRetrospectively analyse the clinical data of ischemic strokeinpatients fromJanuary2011to January2012in neurology department of Yunfu people’s hospital.Classify the enrolled cases consistent with TOAST criteria by two experiencedphysicians and compare the ratio with the subtype of United States, Germany, SouthKorea, Taiwan, Beijing and Chengdu. statistics the quality of key indexes and makecomparison with1staffiliated hospital of Jinan University, Beijing, China, Taiwan andthe United States.ResultsA total of211patients wereenrolled, according to TOAST classification,104cases areSUE（49.29%）,75cases are SAA(35.55%),17cases of LAA(8.06%),14cases of CE（6.64%）,1case of SOE（0.49%）.Through the quality control indexes,7cases(3.3%) received thrombolytic therapy, in which3cases (42.86%)with intra-arterialthrombolysis,4cases (57.14%) with intravenous thrombolytic therapy and3cases(42.86%) occur hemorrhagic transformation.136cases(64.45%) acceptanti-thrombosis therapy within48h after admission,58（26.7%）cases accept statintreatment,2（14.29%）atrial fibrillation patients accept anticoagulant therapy, amongthe patients unable to walk,9case（s8.3%）accept deep venous thrombosis prevention.125cases(59.2%) get prescription with anti-thrombotic drugs at discharge.55cases (26.7%)get prescription with Lipid lowering drugs at discharge.46cases （21.8%）quitted smoking,46（21.8%） patients were educated about stroke. All of the qualityratios are lower than those in China, Taiwan, U.S.A, but the usage of traditionalChinese medicine and brain protein hydrolysate were much higher.Conclusions1、 SUE stands for the highest percentage in the TOAST subtype classification inYunfu people’s hospital, it’s due to shortage of examinationequipments. LAA is lowerthan that of U.S., Taiwan, Beijing and Chengdu. It is necessary to popularize therelated examination to identify the TOAST subtype.2、The ratios of anti thrombosis therapy and stat in treatment within48h afteradmissionand at discharge are close to average level in mainland while other indexeshave great gaps with both of the United States and some of the mainland areas andneeds for further effort.
