Objecitive:(1)To discuss the clinical manifestations difference between incipientacute myocardial infarction patients with low LDL and others;(2)To discuss thestatin benefit difference between incipient acute myocardial infarction patients withlow LDL and others. Methods:The study selected815cases incipient acutemyocardial infarction patients from my hospital and divided them into four groups:the first group:LDL-C <1.81mmol/L (70mg/dl), a total of56people;the secondgroup:1.81mmol/L (70mg/dl)≤LDL-C<2.60mmol/L (100mg/dl), a total of208people. The third group:2.60mmol/L (100mg/dl)≤LDL-C <3.37mmol/L (130mg/dl), a total of73people. The fourth group: LDL-C≥3.37/L (130mg/dl), a totalof478people. Take the first, second and the third group as research group,the fourthgroup as control group. Compare their general situation, biochemical criterion,infarction location, infarction area, Coronary angiography situation, adverse eventsoccurring during the period in hospital and the change of blood lipids, statin intensity,adverse events occurring after statin treatment more than one year, then use theLogestic regression analysis to select the main factors influencing the adverse events.Resulits (1)The research group has greater infarction area, higher percent in singlevasculopathy, lower pencent in multi vasculopathy,lower Gensini garde than thecontrol group;Logestic regression analysis showed that the probability of adverseevents occurring during the period in hospital was positively correlated with the age(regression coefficient t=1.876), Cr level(regression coefficient t=1.147),Occerring extensive anterior wall infarction(regression coefficient t=1.258), Gensinigrade(regression coefficient t=1.176) and negatively correlated with HDL-C level(regression coefficient t=-1.408).(2)After one year statin treatment, blood lipids has only small differences between the research group and the control group;Logesticregression analysis showed that the probability of adverse events occurring after statintreatment more than one year was positively correlated with the age(regressioncoefficient t=0.735), FBG level(regression coefficient t=0.688), occurring multivasculopathy(regression coefficient t=0.547), area of infarction(regressioncoefficient t=0.592) and negatively correlated with LDL-C level(regressioncoefficient t=-0.347).Conclusion:(1)Incipient acute myocardial infarction patientswith low LDL-C level has greater area of infarction,but less serious of coronaryartery lesions;the probability of occurring adverse events during the period in hospitalhas no statistical significance difference from others, the main impact factors are age,HDL-C level, Cr level, occurring extensive anterior wall infarction and Gensini grade.(2)After one year statin treatment, incipient acute myocardial infarction patientswith low LDL-C level has only small difference in blood lipids from others;theprobability of adverse events occurring after statin treatment more than one year ismore than other paitents, the main impact factors are age, FBG level, LDL-C level,occurring multi vasculopathy and area of infarction.
