| Part one: Early structural changes in diabetic patientswithout retinopathy detected by optical coherencetomographyObjective: To observe the early structural changes in Type1and Type2diabetic patients without retinopathy detected by optical coherencetomography.Methods:20(20eyes) Type2diabetes,10(10eyes) with Type1diabetes,who had no retinopathy and accepted treatment from October2013toMarch2014at endocrine departments of Ruijin Hospital were separatelyexamined by optical coherence tomography. Mean retinal thicknessmeasured by optical coherence tomography was calculated for the fovea,the pericentral and the peripheral macula,and compared to healthy controls.The thickness of inner nuclear layer, outer nuclear layer, ganglion cell layerand the thickness of choroid in type1diabetes was obtained in nasal andtemporal areas. Results: Comparing the Type2normal and NDR eyes,retinal thicknesssignificantly decreased in A3, A4, A5(P<0.05). The ganglion cell layerdecreased in nasal (P<0.05). The inner nuclear layer decreased in1.5mm,1.0mm,0.5mm nasal to the fovea and0.5mm temporal to the fovea(P<0.05). In the outer nuclear layer there were no differences betweendiabetics and controls. Comparing the Type1normal and NDR eyes, therewere no differences between diabetics and controls in retinal thickness,inner nuclear layer, outer nuclear layer, ganglion cell layer and choroid.Conclusion: The early changes of retinal structure in Type2diabetic eyeswithout retinopathy can be detected objectively and quantitatively byoptical coherence tomography. Part two: Early functional changes in diabetic patientswithout retinopathy detected by multifocal electroretinogramand oscillatory potentialsObjective: To observe the early functional changes in Type1and Type2diabetic patients without retinopathy detected by multifocalelectroretinogram and oscillatory potentials.Methods:20(20eyes) Type2diabetes,10(10eyes) with Type1diabetes,who had no retinopathy and accepted treatment from October2013toMarch2014at endocrine departments of Ruijin Hospital were separately examined by multifocal electroretinogram and electroretinogram. Theamplitude of OPs was calculated. And a first order response was calculatedin5concentric rings and4quadrant. Response densities of P1wereanalyzed.Results: Comparing the Type2normal and NDR eyes,responses densitiesof P1were lower in rings from2to4and superior-temporal quadrant andsuperior-nasal quadrant and inferior-nasal quadrant (P<0.05). Comparingthe Type1normal and NDR eyes, responses densities of P1were lower inrings from2to4and superior-temporal quadrant and superior-nasalquadrant (P<0.05). Comparing the Type1and2normal and NDR eyes,the amplitude of OPs were significantly lower.Conclusion: The early changes of retinal function in Type1and2diabeticeyes without retinopathy can be detected objectively and quantitatively bymultifocal electroretinogram and oscillatory potentials. |
