Objectives To observe the clinical efficacy of pressure perfusion of ShuxuetongInjection in femoral artery for treatment of grade2diabetic foot, providing relevant clinicaldata for clinical treatment of grade2diabetic foot.Methods120patients with grade2diabetic foot checked in Tangshan Xiehe Hospitalfrom2011to2014were recruited. They were randomly divided into Shuxuetong Injection+pressure perfusion in femoral artery group, Shuxuetong Injection+intravenous injectiongroup, and prostaglandin E1infusion group, n=40, according to the admission number andthe random number table. The results of the clinical examinations before and aftertreatment were recorded, and concluded by statistical analysis.Results1General clinical symptoms: The general clinical symptoms were recordedmainly in indexes including cold limbs, rest pains, intermittent claudication, ABI indexand wound area. Three groups of patients were all improved after treatment, in which theShuxuetong Injection+pressure perfusion in femoral artery group and prostaglandin E1infusion group were better than the Shuxuetong Injection+intravenous injection group(P<0.05).2Blood test: The results suggested that the whole blood viscosity at high shearrate、the whole blood viscosity at low shear rate、plasma viscosity and fibrinogen ofthree groups were all improved after treatment. the Shuxuetong Injection+pressureperfusion in femoral artery group were better than the Shuxuetong Injection+intravenous injection group and the prostaglandin E1infusion group (P<0.05).3Lowerextremity vascular ultrasound examination: The improved blood flow beam width ofposterior tibial artery, superficial femoral artery, and dorsal artery, and improved bloodflow spectrum peak of posterior tibial artery, anterior tibial artery, and popliteal artery inthe Shuxuetong Injection+pressure perfusion in femoral artery group were better thanother two groups (P<0.05).4Wound:Comparision of wound of three groups of casesbefore and after treatment. Shuxuetong Injection+pressure perfusion in femoral arterygroup were better than other two groups (P<0.05).Conclusions1The pressure perfusion in femoral artery can achieve the maximumplasma concentration at the affected area, with less medications and the maximum effect,rapid action, shorten the treatment, alleviate the suffering of patients and easy to operate.It is more effective than intravenous injection for treatment of diabetic foot.2Shuxuetong Injection is made from leech, the TCM compound preparation of earthworm, playing roles of promoting blood circulation to remove blood stasis, and activatingmeridians to stop pains. According to the Western medicine studies, leech and earthwormhave multiple effects including thrombolysis, anticoagulant, anti-inflammatory, analgesic,nerves repair, promotion of nerve regeneration, and promotion of wound healing. Theresults have showed that the clinical efficacy of Shuxuetong Injection is better thanprostaglandin E1injection.3Shuxuetong Injection combined with pressure perfusion infemoral artery has a significant clinical effect on treatment of grade2diabetic foot, and issuitable for clinical practice.
