Analysis Of Prognostic Factors Of Diffuse Large B-cell Lymphoma

Objective: To investigate the relationship between the clinical and pathologicalfeatures and prognosis in patients with diffuse large B-cell lymphoma(DLBCL)．Materials and methods:134cases of patients (who were older than14years old)with DLBCL treated from January2008to December2012in department ofHematology, Fujian Medical University Union Hospital.Diagnosis of patients wereconfirmed by pathological histology.We collected follow up data,recorded everypatient’s age, gender, clinical stages, infiltration, ECOG score, internationalprognostic index (IPI), with or without systemic symptom, LDH,immunohistochemical results, treatments, progression-free survival (PFS) and overallsurvival (OS).Results: The efficacy analysis showed that patients with clinical stage I and II,normal LDH,without B symptom, IPI≤2, ECOG score <2, infiltration area <2, Ki-67positive expression rate≥80%, had better short-term effects of DLBCL. Univariateanalysis showed patients who were at clinical stage I and II, normal LDH, without Bsymptom, IPI≤2, ECOG score <2, infiltration area <2, the bcl-6positiveexpression, achieving CR or PR had better prognosis.But age, germinal center, theBCL-2, CD10, CD5, Mum-1, Ki-67positive expression rate were not prognositicfactors of DLBCL. R–CHOP like treatment group is better than that of CHOP liketreatment group on the chemotherapy remission rate and overall survival, but thedifferences in the efficacy analysis and univariate analysis were not significant.Multivariate analysis showed that the bcl-6positive expression was the independentprognositic factor of DLBCL.The prognostic significance of some indicators wasdifferent from most analysis abroad, which may be associated with fewer cases.Conclusion: The prognosis of DLBCL has a certain relationship with stages, IPIscore, ECOG score, LDH, systemic symptom, and number of involvement，the BCL-6expression and achieving CR or PR.
