| Objective:The purpose of this study was to compare the efficacy of Stereotactic body radiation therapy and surgical treatment of early-stage non-small cell lung cancer. Method:By searched the Cochrane Library, PubMed, Medline, Chinese Journal Full-text Database (CNKI), Chinese Biomedical Literature Database (CBM) and Wan Fang DATA, collection of published literature about SBRT and surgical treatment of early-stage NSCLC at home and abroad, taken double parallel evaluation method to evaluate collected literature, extracted the data and cross checked, meta-analyses were performed by RevMan5.2software. Results:Four retrospective studies involving1265cases were included, surgical treatment of914cases and SBRT351cases,it can be divided into A, B, C three groups to analysis the survival of two groups, Group A:4Studies on comprehensive evaluation of two treatments survival; Group B:two studies two groups cases were1:1matched, there was no statistical significance in age, comorbidity score and tumor staging in two groups; Group C:another two studies two groups cases were1:1matched, there was statistical significance in age, comorbidity score and pulmonary function in two groups.Group A:1-year overall survival difference had no statistical significance(P=0.01),2-year and3-year overall survival difference had statistical significance(P<0.00001;P<0.00001), surgery group better than the SBRT group; Group B:1-year,2-year overall survival difference was not statistically significant(T=0.35; P=0.29),3-year overall survival difference was statistically significant(P=0.02), surgery group better than the SBRT group; Group C:1-year,2-year and3-year overall survival difference had statistical significance(P=0.002; P<0.00001; P<0.00001), surgery group better than the SBRT group; There was no statistically significant difference on two groups tumor-specific survival rate(P=0.46, P=0.19). Conclusion:Surgery group compared with SBRT group overall survival, due to differences in age, pulmonary function and complications between two groups, we cannot clearly drawn conclusions SBRT group better than surgery group, But tumor-specific survival rate between the two groups have no difference, therefore, the comparison of curative effect of two groups need randomized controlled trials to be further explored. |
