Objective:Risk factors for amputation, ulcer grade and management outcome of diabetic foot ulcers.Methods:227DF patients were dicided,based on whether receiving limb amputation during hospitalization,into goop non-amputation(n=162) and amputation(=64).The clinical features and laboratory test results and their relationship to amputation were analyzed.Results:Amputation group of patients with age, duration of hospital stay, systolic blood pressure, white blood cell count, hematocrit, fibrinogen, diabetic peripheral neuropathy, diabetic hyperosmolar state/diabetic ketoacidosis, diabetic peripheral vascular disease, total protein, infection, the incidence of ulcers, Admission serum glucose,2hour postprandial serum glucose, glycosylated hemoglobin above were significantly higher than those in non amputation group; The two group Age, days of hospitalization, SBP, FBG, PBG, WBC, FIB, HbACl and complication of diabetic peripheral neuropathy, diabetic peripheral vascular disease, diabetes ketoacidosis/diabetic hyperosmolar state indicators have significant difference (P<0.05), Logistic analysis showed that FIB, SBP, diabetic peripheral neuropathy, diabetic peripheral artery disease is an independent risk factor for type2diabetes foot. Diabetic foot patients according to Wagner classification of different, the difference was statistically significant DF amputation rate (P<0.05); Wagner grade Ⅱ-Ⅲ Wagner IV amputation rate is low, significantly higher than that of0~Ⅰ.Diabetic foot amputation group local tissue bacterial culture positive cases were significantly higher than those in non amputation group, DF infection rate were statistically significant, multiple infection was significantly higher than that of single bacteria species, bacillus, coccus, fungi, difference has statistics significance. Logistic analysis showed that the diabetic foot and the risk factors related to the occurrence of FIB, SBP, PAD, DPN had statistical significance (P<0.05), age, gender, FBG, DN, coronary heart disease, the diabetic foot increased relative risk (OR>I), but statistical significance (P>0.05), HDL-c is one of the protection factors, but statistics there is no difference (P>0.05).Conclusion:The early education of diabetes disease, screening for diabetic neuropathy, strengthen the long-term vascular lesions, strict control of blood glucose levels, and the regulation of fibrinogen, blood pressure, on foot disease prevention is heavy, urge patients to treatment as soon as possible to reduce the cost of medical care, in order to reduce the occurrence of amputation.
