The Clinical Research On The High-order Position Perianal Abscess Treated By Catheter Drainage Method

Objective:By investigating the high-order position perianal abscess treated by double lumen catheter drainage method and negative pressure treatment,compared with simple drainage or seton drainage, and observing the clinical efficiency,postoperative complications and postoperative follow-up,in order to explore a better long-term result of treatment,fewer complications after minimally invasive surgical method.Method:This study adopts the method of clinical observation, select40cases of high-order position perianal abscess diagnosed by endo-luminal ultrasound. These cases are randomly divided into research group and the control group (n=20).The research group adopt double lumen catheter drainage method and negative pressure treatment.The control group adopts simple drainage or seton drainage.Under the general information on the clinical aspects(age,gender,pathogenetic condition,pathogenesis)were comparable,Observe:1. The curative effect;2. The length of the incision;3. The VAS pain score points of2to7days after operation;4. The final wound healing time;5. The wexner anal incontinence score points of the first,third and sixth month after operation;6.Review fistula rate and recurrence rate by endo-luminal ultrasound after1,3,6month.Results:Followed up for six months, the research group recurrence in2cases, anal fistula formation in3cases, the average length of incision is3.22±0.94cm, the final wound healing time is20.60±4.10days, the VAS pain score points of2-7days after operation is15.55±3.98points,the wexner anal incontinence scores points of the first,third and sixth month after operation is0.05±0.22points.The control group3cases relapse, anal fistula formation in5cases,the average length of incision is7.53±2.22cm, the final wound healing time is53.80±10.51days, the VAS pain score points of2～7days after operation is30.85±3.12points, the wexner anal incontinence scores points of the first,third and sixth month after operation is0.95±1.23points.No Statistical difference existed in the two group of overall curative effect and the risk of perianal abscess recurrence and anal fistula(P>0.05).Statistical difference existed in the two group of the length of incision,the final wound healing time,the treatment pain,the anal incontinence scores,the risk of complexity perianal abscess recurrence and complexity anal fistula formation (P<0.05).ConcIusion:The research group adopt double lumen catheter drainage method and negative pressure treatment is smaller than control group in the incision,less pain,shorter healing time,better anal function protection,lower risk of complexity perianal abscess recurrence and complexity anal fistula formation.The method of double lumen catheter drainage method and negative pressure treatment is better operation method in the clinical application of high-order position perianal abscess.
