Objective To study the influence of the proton pump inhibitors (PPI)on the process and prognosis of hospital-acquired pneumonia (HAP)patients in intensive care unit (ICU).Methods This study focused on the comparative analysis of theprocess and prognosis situation between the patients with and without PPItreatment. The comparative analysis was performed based on the clinicalinformation of the past HAP cases in which the patients were hospitalizedin the ICU and were in the danger of aspiration form the an affiliatedhospital of a medical college between Jan2011and Dec2011. And then theinfluence of the PPI over these cases was investigated according to thedifferent doses.Results A total of65cases of HAP were investigated in this study. In45cases, the PPI was used, while in the rest of20cases noacid-suppressive drug was used during the disease course. Baseline surveyis that both the basic diseases and indicators of various infections were not statistically different. There were more patients with bloodstream infectionsand abnormal test results of temperature, leukocyte count, oxygenationindex, arterial blood gas pH value and A-aDO2, compared with the controlgroup after comprehensive treatment for1week. Inaddition, it wasobserved that the scope of infect of CT had been widended in PPI group, inwhich the anti-infective theapy was more ineffective the28days mortalitywas higher. The data mentioned above are statistically difference betweenthe two groups(P<0.05). However, the result showed that there was nodifference (P>0.05) between the cases in which different defined dailydoses (DDD) were applied.Conclusion There is a considerable chace that using the PPI wouldnot be beneficial to the pneumonic infection of HAP patients in ICU andworsen the prognosis situation. Therefore the clinical application of PPI toHAP patients should be made with cautiousness.However,there is nodifference in different DDDs of the PPI applications.
