Objective: Research on influence of (Pancreaticoduodenectomy,PD)resection related factors of postoperative pancreatic leakage occurs,to guideclinical prevention and treatment of pancreatic leakage. Methods: Retrospective analysis of heavy physician a hospitalbetween January2011and October2011line between PD patients datacomplete cases61cases.Analysis of patients with postoperative pancreaticleakage and the existence of the following factors, mainly including thefollowing three aspects:(1)the preoperative examination results: gender, age, preoperative bilirubin, preoperative albumin and preoperative cerealthird transaminase;(2)intraoperative related factors:whether the pancreaticduct diameter size, pancreatic duct support drainage tube placement, operation method, the types of pancreatic stump and digestive tractreconstruction in, how much the length of the operation time andintraoperative blood loss and pancreas texture, etc. Results:In January2011to October2013,our hospital had61cases ofcomplete record of the regular PD.Postoperative pancreatic leakage,11cases(14.03%),including4cases died(6.56%),after the second surgeryhealing in2cases,the rest of the conservative treatment to heal.Statisticalanalysis showed that patients with gender, age, preoperative albumin, preoperative cereal third transaminase, pancreatic stump and digestive tractreconstruction method and operating time had no obvious correlation withthe incidence of postoperative pancreatic leakage (P>0.05);high preoperative bilirubin>171umol/L,the diameter of the pancreatic ductfine<3mm,pancreatic duct was not placed supporting tube, intraoperativeblood loss>1000ml and postoperative unused albumin(Human serumalbumin, HSA), the incidence of postoperative pancreatic leakagesignificantly higher(P<0.05), there were significant difference. Conclusions: The occurrence of postoperative pancreatic leakage hadobvious correlation with preoperative liver function, intraoperative factorsof operation and postoperative treatment measures. Meticulous and stricttreatment with surgery and postoperative reasonable measures wereimportant means to prevent or reduce postoperative pancreatic leakagehappened.
