ObjectiveObserving the changes of NGAL in T2DM patients complicated with DN disease, to explore the clinical significance of serum NGAL expression level at the early stage of T2DM nephropathy, and combined to detect the serum NGAL and the levels of hs-CRP, CysC, Urea, SCr, and24hUAER commonly used nowadays, compared their clinical value in the early diagnosis of T2DM patients complicated with DN disease.MethodsThrough clinical history, collecting the cases of T2DM having been diagnosed, blood sugar and blood pressure had stably controlled in recent one month, and excluded the other primary disease outside the diabetes through the regular examination and clinical observation (such as primary hypertension, primary kidney disease, the immune system and other related diseases), referring to the Morgensen diabetic nephropathy staging criteria, I selected110cases of T2DM patients in our hospital endocrinology, on the basis of24hours urinary albumin excretion (UAER) T2DM subjects can be divided into three groups:(1) the group A (SDM):UAER<30mg/24h,(2) the group B (EDN):UAER30-300mg/24h,(3) the group C(CDN):UAER>300mg/24h. And select the same period in our hospital cheek-up of40cases of healthy subjects as control group. Testing groups according to the requirements of related SOP file NGAL, hypersensitive c-reactive protein and serum inhibition C, urea nitrogen, creatinine, and24h urine micro protein levels.Results Compared by group of control, serum NGAL, Urea, Cr, CysC, hs-CRP, and UAER in simple DM group, change was not significant (P>0.05); Serum NGAL of diabetes early renal injury group, CysC,hs-CRP level a little elevated (P<0.05), while SCr, Urea had little significant change (P>0.05); the difference was statistically significant (P<0.05), the detection positive rate than that of urea, Cr, positive rate was significantly increased. With the increase of UAER, elevated serum level of NGAL will increase (P<0.05), and its application in T2DM diagnosis positive rate and early renal damage in inhibition, hypersensitive c-reactive protein C.Conclusion1. Serum NGAL can serve as a kind of new biomarkers of kidney injury,which may rise obviously in the early renal damage in type2diabetes, and earlier than Scr, serum Urea, more sensitive to reflect diabetes early renal function changes, can be used as a diagnosis of diabetes one of sensitive indexes of early renal damage.2. Serum NGAL and hs-CRP、CysC had higher positive detection rate in early renal damage of type2diabetes, the positive rate was significantly higher serum urea and creatinine measurements.And combined detection can greatly improve the positive rate of diagnosis.Early kidney disease in type2diabetes screening and evaluation of early renal damage is important for clinical significance.3. NGAL and hs-CRP、CysC、UAER were positively correlated but generally correlated with Urea、SCr, so in several check if clinical joint, can obviously improve the early diagnosis of positive, if found to have two or more in clinical renal damage index increases, and serum urea and creatinine increased not seen in patients with type2diabetes must pay close attention to, intervention and effective treatment as soon as possible, for development of type2diabetic kidney disease and prognosis have important clinical significance.
