Clinical Features Of Types Of Multiple Myeloma And Relationships With Cytogenetic Abnormalities

Object:To summarize the clinical features of newly diagnosed multiple myeloma, To investigate the clinical significance of the chromosomal changes in MM patients,and relationships with clinical features (including responses to therapies)Method:Records of90MM patients in whom MM was initially diagnosed at the Xiangya hospital of Central South University, from May2012to January2014were reviewed, to analyze clinical indexs (Hemoglobin, serum creatinine and so on). Fluorescence in situ hybridization (FISH) on bone marrow (BM) cells was performed in40enrolled MM patients. The relationships between chromosomal abnormalities and clinical features were analyzed.Results:ISS Ⅲ is the most common clinical staging; Ostealgia is the most common symptom of IgG、 IgA and light-chain MM (59.5%、60.7%、61.5%、71.4%、67.86%、61.5%).There is no significant difference in the percentage of plasma cell in bone marrow、absolute lymphocyte count、Hemoglobin、albumin、serum creatinine> level of β2-microglobulin、serum creatinine and Cys C(p>0.05) between light-chain MM and IgG MM,IgA MM and IgG MM, light- chain MM and IgA MM。 There is significant difference in LDH (p<0.05) between IgA MM and IgG MM, light-chain MM and IgA MM。As a result of FISH,72.5%(29/40) patients had chromosome changes.The incidences of each probe were13q deletions35%,17p deletions12.5%, and chromosome1q amplification45%. the most common clinical staging of these patients is ISSⅢ(78.6%,80%,66.6%).Some cytogenetic abnormalities were correlated to clinical features. Spearman correlation analysis were found associated of17p-with β2-MG、LDH、sCr (r=0.93P=0.02; r=0.88P=0.04; r=0.95P=0.01),amp(1q21) with albumin(r=-0.561, P=0.037). Treatments with bortezomib gave patients a notably higher ORR rates, but1q21amplification,13q deletions not.Conclusion:1. ISS Ⅲ is the most common clinical staging and Ostealgia is the most common symptom of newly diagnosed IgG (59.5%、71.4%)、 IgA(60.7%,67.86%)、light-chain MM (61.5%.61.5%).2. There is significant difference in LDH(p<0.05) between IgA MM and IgG MM, light-chain MM and IgA MM,suggesting tumor activity of IgA MM may lower than others.3. A cytogenetic aberration was found in most of MM paients (72.5%) at least. Cytogenetic abnormalities were correlated to clinical features、prognosis and patients’responses to therapies.
