[Objective] To observe the clinical efficacy of low molecular weight heparin in treatment of severe sepsis.[Methods] twenty-two sepsic patients’ medical history date and lab index in our emergency intensive care unit from Octomber2012to April2013were collected. Severe sepsis patients were divided into two group, eleven patients were given the traditional treatment, and the another eleven were given heparin include the traditional treatment. The heparin group was percutaneously injected with heparin twice a day for7days based on the traditional therapy in the tradition group. APACHE Ⅱ scores, CRP, WBC, PLT, APTT, PT, FIB andD-D were detected before and after treatment. And28-day mortality were calculated in both groups.[Results]①Before treatment, the heparin group APACHE Ⅱ、WBC and CRP (separately19.09±3.58、20.75±4.81×109/L、165.88±44.57mg/L) were compared with the traditional group APACHE Ⅱ、WBC、CRP (separately18.00±3.74、19.87±4.07×109/L、144.55±41.15mg/L). There was no significant difference between two groups (P>0.05).After be treated7days, the heparin group APACHE II、WBC and CRP (separately9.36±2.58、9.17±3.20×109/L、55.15±8.14mg/L) were compared with the traditional group APACHE Ⅱ、WBC and CRP (separately14.45±6.28、13.78±2.45×109/L,73.91±18.9mg/L), and the heparin group was much lower than the control group,(P<0.05);②Before treatment, the heparin group PLT (238.86±132.76×109/L) was compared with the traditional group PLT (177.81±77.11×109/L), and there was no significant difference between each other,(P>0.05). After be treated7days, the heparin group PLT (335.54+89.75×109/L) was compared with the traditional group PLT (153.46±82.00×109/L), the heparin was much higher than the traditional one,(P<0.05)③Before treatment, the heparin group APTT、PT (separately31.79±4.96s、15.42±2.48s)were compared with the traditional group APTT、PT(32.73±4.80s,15.35±1.94s) and there was no significant differences between each other (P>0.05).After be treated7days, the heparin group APTT、 PT (separately30.83±4.53s,14.36±2.75s) were compared with the traditional group APTT. PT (31.58±5.54s.15.62±1.57s)and there was no significant difference between each other (P>0.05);④Before treatment, the heparin group FIB (404.27±140.72mg/dL)was compared with the traditional group FIB (407.55±190.96mg/dL) there was no significant difference between each other (P>0.05). After be treated7days, the heparin group FIB(470.00±148.77mg/dL) was compared with the traditional group FIB(279.00±159.65) the heparin was much higher than the traditional one (P<0.05);⑤Before treatment, the heparin group D-D(10.14±9.20mg/L)was compared with the traditional group D-D (9.89±11.50mg/L) there was no significant difference between each other (P>0.05).After be treated7days, the heparin group (4.23±3.38mg/L) was compared with the traditional group DHD(9.04±11.77mg/L)and there was no significant difference between each other (P>0.05).⑥The traditional group28-day mortality about36.36%was compared with the heparin group28-day mortality about27.27%, and there was no significant differences between each other (P>0.05).[Conclusion] Low molecular weight heparin could attenuate inflammation and reduce the incidence rate of PLT losing and has no effect on intrinsic and extrinsic coagulation systems. On the other hand, low molecular weight heparin can improve the fibrinolytic system...
