Objective:To explore the level and clinical signficance of serum ferritin(SF)in myelodysplastic syndrome(MDS) patients with different subtypes.Methods:SF was measured by electrochemiluminescence immunoassay in103cases MDS patients and100cases healthy control groups,and the level of SF and the relationship with International Prognostic Scoring System (IPSS) was observed. Prussian blue staining method is used to determine the extracellular iron and intracellular iron,to investigate the relationship between SF and extracellular iron and intracellular iron.58patients were followed up and divided into two groups(SF<500ng/ml and SF>500ng/ml) according to the level of SF,and the time of transformed into leukemia and survival was compared. Results:The SF level was significantly higher in MDS patients than in healthy control groups(P<0.05),but the SF level between each subtypes of MDS showed no statistical significance(P>0.05). SF and extracellular iron was significantly positively correlation in every MDS patients (P <0.05). SF showed statistical significance between the relatively low-risk group and the relatively high-risk group(P<0.05).The SF level in the relatively high-risk group were obviously higher than the relatively low risk group,and the SF level in danger-Ⅱ group is higher than the other three groups.14cases of the followed up58patients were developed into acute leukemia. The leukemic transformation rates were22.58%and25.93%of the two groups,and the median time of leukemic transformation were10(7,21) and5(2,6) months. The time of leukemic transformation between the two groups was significantly (P <0.05),but the time of survival was no significant (P>0.05).Conclusion:Non-transfuscd MDS patients have iron overload. SF can be used as one of the indicators in monitoring the MDS patients with iron overload,so it has a certain significance on prognosis.
