Objective:To investigate the features of the intracranial epidermoid cyst imaging and the pros and cons of the cerebellopontine angle epidermoid cyst operative method. Methods:To analyze retrospectively of clinical data of21cases of epidermoid cysts from March2012to January.21patients (including15cases of cerebellopontine angle epidermoid cyst) were treated by microsurgery.10cases of cerebellopontine angle epidermoid cyst were conducted by neuroendoscopy assisted microneurosurgery from January2007to December2009.Rusults:17cases (77.3%) showed a typical manifestation on imaging, while4cases (22.7%) showed a special performance among the21cases of epidermoid cysts.15cases were located in the cerebellopontine angle,1case in the middle cranial fossa,1case in the right temporal lobe,1case in the cerebellar vermis,1case in the left frontotemporal,1case in the left side of the orbit,1case in the fourth ventricle. Total resection was achieved in17cases (81.0%), while subtotal resection in4cases (19.0%).1cases died. There were no recurrence during the postoperative follow-up in the period3months to1year. There was no difference about whole cut rat (P=0.626>0.05) and postoperative neurologic dysfunction rate (P>0.05) between the two groups.Conclusion:The majority of epidermoid cysts on the CT showed a low density, while on the MRI they showed a long T1WI and long T2WI signal and the DWI showed a high signal with no enhancement after scanning. However, the diagnosis of some cases with atypical imaging performance in uncommon position was more difficult., and the clear diagnosis often needed the intraoperative findings and postoperative pathology. Endoscopic assisted microsurgery enlages surgical field and eliminate the blind spots in the surgery. At the same time it improve the understanding of the total resection rate and does not affect facial nerve function.
