Objective:To understand arsenic content in high-arsenic area in Kuitun of Xinjiang and health status of local residents currently, explore the damage effect on human liver induced by arsenic and provide the basis for further studying arsenic mechanism of the liver injury. Methods:Meta-analysis was used to investigate the correlation between endemic arsenic poisoning and liver injury, as a theoretical basis for the further investigation at the scene. Review the result of monitoring of high arsenic and analysis arsenic content changes and distribution status in previous years in KuiTun. According to the testing results of arsenic, the residents of129regiment exposure high arsenic water were choosed as the case group,129local residents (water arsenic levels below0.01mg/1) as the control group, the adjacent125regiment without arsenic exposure as control group. Make questionnaire survey (basic situation, urine arsenic test, dermatologist check, B ultrasonic examination, etc.) to them, collect questionnaires, sample collection and preservation, etc. Determinate biological sample and the collected data were entried into Epidata, apply SPSS19.0to analyse the data. Results:1.The research whether endemic arsenic poisoning and liver damage is related through meta analysis heterogeneity test, P=0.16, appropriate to use fixed effect model, and showing the results that the arsenic exposure and liver damage is moderate related(OR=3.72), indicated arsenic exposure may be a risk factor of high incidence of liver disease in arsenic poisoning area. In view of arsenic exposure group liver injury occurrence possibility is higher than that of control group, so the arsenic exposure may be one of the main reasons for high incidence of liver disease in endemic area.2. Epidemiological research:On the basis of analysis of monitoring results in recent years, detecting groups of arsenic in water, this survey found that water arsenic exposure area exceeding standard, water arsenic exposure group and the internal and external control group difference was statistically significant (P<0.05). Base on the crowd epidemiological data, selected200cases from exposed group and100cases internal and external control group respectively, found that the comparison between internal and external controls of urinary arsenic content difference was statistically significant while there was no statistically significant difference between the internal and external control group. The liver B ultrasonic inspection showed that difference of liver injury between exposed group and internal and external control group was statistically significant respectively. Liver function markers detection showed that difference of serum hyaluronic acid, laminin, IV collagen, between exposed group and internal and external control group was statistically significant respectively. Difference of type III before collagen peptide between exposed group and internal control group was no statistically significant, while it is statistically significant between exposed group and external control group. Compared with the internal control group and external control group showed difference of laminin and collagen type IV is statistically significant. Conclusion:Exposed people are prone to get skin, spirit, system diseases and liver damage, and also serum liver fibrosis indexes of exposed group are higher than the control group, indicating arsenic exposure are associated with a high incidence of liver cirrhosis, arsenic exposure is likely to be one of the reasons lead to liver damage, especially liver fibrosis.
