Comparative Analysis Of Curative Effects Of Unilateral And Bilateral Internal Fixation In Treatment Of Lumbar Disc Herniation

Objective:To compare the curative effects of unilateral and bilateral internal fixation in the treatment of lumbar disc herniation in order to provide clinical reference.Methods:From January2012to April2013, a total58cases of lumbar disc herniation with surgical treatment of lumbar interbody fusion who have complete postoperative follow-up, there were28male and30female with the mean age of55.13years (from27to78years old), the follow-up period from6to22months(average16months). All the patients were divided into group A and B according to different surgical approaches. The28cases in group A were treated with unilateral pedicle screw fixation by the paramedian approach. The30cases in group B were treated with bilateral pedicle screw fixation by the traditional posterior median approach. The parameters include ODI, JOA, VAS scores, Stauffer Coventry Scale, intraoperative blood loss were analyzed to comparative postoperative efficacy and lumbar function improvement..Results:Statistically significant differences of the operation time, intraoperative blood loss, and hospitalization expenses were found between the two groups (P<0.05). No screw loosening, displacement, breakage and cage displacement or settlement was found during the follow-up period. At the final follow-up, there were no statistical difference in ODI, JOA,VAS score and Stauffer Coventry Scale were found between the two groups (P>0.05).Conclusion:Compared with the bilateral pedicle screw fixation, the unilateral pedicle screw fixation can also get the satisfactory curative effect, and also better in easily operative, less intraoperative blood loss, less operation time and lower hospitalization expenses, thereby worthy of promotion and application.
