Objective:With the guidance of the theory of TCM, on the ground of in tutor formerly clinical study and the guidance of the professor, to investigate the relationship between vital Qi deficiency mechanism and T-lymphocyte subsets characteristic in HBV-ACLF. To investigate HBV-ACLF patients’immunological change after the treatment of supplementing Qi and tonifying spleen by means of observing the change of T-lymphocyte subsets frequencies.Methods:60patients with HBV-ACLF, collected on December1,2012to January31,2014from the second hospital of Nanjing liver disease inpatient. All of the patients’diagnosis must be in accordance with diagnostic criteria and inclusion criteria. Collect the patients’basic information, installment, TCM syndromes and other information to analyze the empirical and deficiency syndrome. All patients were treated with conventional medical therapy, the treatment group was treated joining Chinese decoction orally based on syndrome differentiation. Observe the patients before and after treatment on related symptoms integral, biochemical index, frequency change of T-lymphocyte subsets, carry on statistics processing, main effect index and record the related data. Compare the index of the two teams to speculate the effective of traditional Chinese medicine to regulate the body symptoms, immunity and the prognosis.Result:The syndrome of HBV-ACLF was involved eight, the most of them are wet syndrome and Qi deficiency syndrome, both of them are accounted for100%, Followed by blood stasis syndrome (58.33%), heat syndrome (51.67%), cold syndrome (48.33%), blood deficiency syndrome (28.33%), Yin deficiency syndrome (25%)and Yang deficiency syndrome (6.67%). The proportion of wet syndrome and Qi deficiency syndrome is higher than other syndromes, the difference was statistically significant (P<0.05). After treatment, the recovery of the treatment group (70.0%) is higher than the control group (43.3%), showed that after joining traditional Chinese medicine have certain improvement to the symptoms and prognosis. The T lymphocyte number of treatment group apparent trends to be higher than the control group, combined with the biochemical examination, blood coagulation function and prognosis, which suggests that the immune function of patients with traditional Chinese medicine has improved.Conclusion:The TCM syndrome type distribution in Patients with HBV-ACLF is given priority to qi deficiency, and Merge other type of syndromes. The frequency of T lymphocytes in HBV-ACLF is related to the prognosis of patients, T lymphocyte frequency increases, the prognosis is relatively good. The difference between before and after the treatment, the treatment group T lymphocyte frequency is higher than the control group, which suggests that traditional Chinese medicine has a regulating for the patient’s immune function.
