Efficiency Analysis And Evaluation On The Quality Of Health Service Of Maternal And Child Health Care Centers In Districts And Counties Of Chongqing

Objective To provide reference for the maternal and child health resourceallocation based on the analysis of resource allocation and state of operation.To provide reference for enhancement of maternal and child health ageciesby evaluating the efficiency with the method of DEA. Using comprehensiveevalution methods to evalute health service quality of maternal and childhealth agecies and provide reference for improvement of health serviequality.Methods The subjects of this study included maternal and child healthinstitutions of37districts and counties in Chongqing. Descriptive statisticswas used to analyze the resource allocation and state of operation. Mutiplestatistical methods were used to select input and output indicators base onliterature review. And then DEA was used to evaluate and analyze theefficiency of maternal and child health institutions. Finally, RSR, entropyevaluation method, TOPSIS, principal component analysis and factoranalysis were used to evalute health service quality of maternal and child health institutions.Results1. The resource allocation of district maternal and child health agencies inChongqingThe level of congfiguration of maternal and child health resources inone-hour economy circle is highest, and in north east and south east wing ofChongqing is lower. The allocation of per-capita resource is highest in mainurban area and one-hour economy circle, the lowest in north east and southeast wing of Chongqing. The quality of human resources in north east andsouth east wing of Chongqing were low. Most maternal and child healthinstitutions in Chongqing lacked public health workers.2. The state of operation of maternal and child health institutions inChongqingThe ouput of medical service is higher in one-hour economy circle andlow in other three regions. The level of public health service in main urbanarea and one-hour economy circle is higher than north east and south eastwing of Chongqing.3. Efficiency analysis of district maternal and child health institution inChongqingA total of eighteen maternal and child health institutions were lessefficient. Institutions in main urban area were more efficient than thoses inother three regions. The proportion of scale efficiency institution in one-hour economy circle is lowest. According to the outcome of input and outputslacks, most institution had lacked pubilc health ouput.4. Comprehensive evaluation of public health service qualityThe results of different methods were consistent. According to theresult of combination evaluation, the level of public health service quality inmain urban area and one-hour economy circle which is economicallydeveloped areas is higher than north east and south east wing of Chongqingwhich is economically undeveloped areas.ConclusionsThe resource allocation is different in different regions. Theinfrastructure construction of maternal and child health institution in mainurban area and north east and south east wing of Chongqing should beenhanced. And public health workers in every county should be increased toimprove the level of public health service.Maternal and child health institutions in main urban area are moreefficient than those in one-hour economy circle and north east wing ofChongqing. For those institutions with low technical efficiency,policymakers should optimize use of resources and avoid wasting resourcesto maximize the output.Maternal and child health institutions in one-hour economy circle arelarger than others. Many insitutions in that area are in the decreasing returnto scale. The scale of institutions in the decreasing return to scale should be controled.Most of those institution with low technical efficiency have less publichealth service ouput. For the counties in north east and south east wing ofChongqing which is economically undeveloped areas, the function of publichealth should be strengthen and maternal and child health services should beincreased to improve the quality of life of women and children.
