Background:Gaomo is a drug made from paste as a medium, a kind of massage applied in certain parts of the body surface or point, drug help to improve the effect of the treatment. It is a combination of drugs and manipulation, effect phase to phase in pharmacy, is a very unique way in the traditional Chinese medicine massage therapy in the treatment of osteoarthritis of the knee. Notopterygium as a medicine to Qufengchushi and pain relief, used to treat cold and dampness, limb pain has been for thousands of years. Notopterygium as the main drug medication and external application in clinical practice can significantly relieve joint synovitis, osteoarthritis of the knee to reduce swelling and pain.But its mechanism of action is unclear. So we volatile by extraction rhizoma et radix notopterygii main component oil, prepared rhizoma et radix notopterygiigaomo cream, observed papain-induced rabbit knee osteoarthritis models synovial inflammatory factors changes and pathological changes to explore rhizoma et radix notopterygiigaomo paste the possible mechanism.Objective:To observe the papain-induced rabbit osteoarthritis of the knee model knee synovial tissue of nitric oxide (NO), interleukin-1(IL-1), metalloproteinase-9(MMP-9) and pathological change, and explore the possible mechanism of action Notopterygium cream rub ointment on the acute phase of osteoarthritis of the knee synovitis.Methods:40experimental animals according to a randomized block design divided into five groups, blank group (n=8), model group (n=8), massage group (n=8), gaomo treatment group (n=8), dressing group(n=8). Osteoarthritis of the knee performed modeling using intra-articular injection of papain. In addition to the blank group of32rabbits at the beginning of the experiment on the day of the first, forth, seventh, in rabbit left knee intra-articular injection of0.5mL volume fraction of2%aqueous solution of papain. The7th day after the last injection modeling side of the knee and the contralateral knee controlled study, the use of Lequesne MG knee assessment method based on the modeling side, local reactions, gait, joint swelling, joint activities to determine the successful model. Blank group and model group untreated, massage group, gaomo group and the dressing group in the last7days after the injection of the drug to be massage、gaomo、dressing rhizoma et radix notopterygiigaomo paste the start of treatment. All animals were sacrificed11days after the start of treatment, cut infrapatellar fat pad at the synovial0.2g Shredded homogenate was centrifuged, and the supernatant, nitrate reductase and protein determination of NO were measured by ELISA IL-1, the use of semi-quantitative immunohistochemical analysis determination of MMP-9, and then through the pathology observed in each group of experimental animals left knee joint synovial tissue inflammation, synovial hyperplasia and vascular proliferation.Results:Lequesne MG knee assessment during the pre-treatment of the blank group with massage group, model group, gaomo group, the dressing group compared significant difference, massage therapy group after treatment in local reactions, gait, joint activities and jointswelling improved significantly, the dressing group improved significantly in terms of local reactions and gait, gaomo group improved significantly in the local reaction, gait and joint swelling; The NO content in model group and dressing group is above in the massage group and gaomo group (P<0.05), the NO content in the massage group and gaomo group is higher than the blank control group (P<0.05). NO content of the model group and treated group was no statistically significant difference (P>0.05), massage group and gaomo group NO content was no Statistically significant difference (P>0.05). The content of the IL-1in Model group, massage group, gaomo group is higher than the blank control group (P<0.05). The content of IL-1in the dressing group and blank group is no statistically significant difference (P>0.05). The content of IL-1in group gaomo IL-1has no statistically significant difference between the massage group, dressing group and model group (P>0.05). Pathological observation and MMP-9semi-quantitative immunohistochemical analysis, no statistically significant difference between the massage group, dressing group, gaomo group and model group (P>0.05).Conclusion:The study found that use the massage and gaomo methods can improve the symptoms of rabbit knee osteoarthritis local reactions, gait, joint activities and joint swelling significantly, and also can reduce the NO content of rabbit knee osteoarthritis synovial. But did not find in the determination of IL-1and MMP-9in the massage, gaomo and dressing statistically significant reduction indicators. Rhizoma et radix notopterygiigaomo paste has a therapeutic effect, but also found in the experiment, some allergic reactions in the dressing group and gaomo group, rhizoma et radix notopterygiigaomo paste itself may have a sensitizing effect, and does not reach the drug and massage phase combined to produce the intended effect of phase to phase to make.
