Objective:The relationship between humoral immune response and anterior chamber inflammatory reaction after phacoemulsification was studied and the role of anti-alpha-crystallin antibody in anterior chamber inflammation was further explored.Methods:①8rabbits were randomly selected from40rabbits as control group (the eyes were obtained preoperatively), and32rabbits were treated as experimental group. The experimental group was treated with phacoemulsification. The rabbits after surgery were divided into4groups with group A (The eyes were obtained1day after surgery), group B (The eyes were obtained one week after surgery), group C (The eyes were obtained two weeks after surgery) and group D (The eyes were obtained one month after surgery). Aqueous flare and corneal edema of the rabbits were examined through slit lamp, and IgG in cornea and iris was tested by Immunofluorescence. IgG in aqueous fluid of all the groups are detected on unoperative eyes and that on one day, one week and two weeks and one month by ELISA.②40rabbits were treated with phacoemulsification. The aqueous humor samples were obtained preoperatively and24hours, one week, two weeks and one month after the operation respectively. Anti-alpha-Cristallin antibody in aqueous fluid was detected by indirect ELISA, and the level of IgG in aqueous humor measured by immunoturbidimetry. Results:①the Mean Optical Density(MOD):Control Group:Cornea:388.00±11.90,Iris:1.04±0.23;Group A:Cornea:470.60±105.07, Iris:177.70±61.66;Group B:Cornea:402.40±19.59,Iris:42.41±3.42;Group C:Cornea:410.70±19.70,Iris:5.15±0.61;Group D:Cornea:396.02±18.425,Iris:1.34±0.37.The IgG in iris of all experimental groups is higher than control group, but there was no significant difference between experimental group4and control group shown by statistical analysis. The IgG in cornea in each experiment group shows no significant difference from that of control group, but immunofluorescence intensity of IgG in severe edema cornea is much higher than others. IgG in the group A are higher than that in contrast group and other experimental group.②IgG in aqueous humor(mg/dl):Preoperation3.46±1.04, one day after surgery8.24±2.56, one week after surgery4.68±1.32, two week after surgery4.22±1.08, one month after surgery3.68±1.02.The IgG in aqueous humor raised after the operation,and reached the nomal level on one month after surgery.Anti-alpha-crystallin antibody was found only among6rabbits suffering severe corneal edema and significant aqueous flare one day after phacoemulsification.Anti-alpha-crystallin antibody was found in1rabbit with severe aqueous flare one week after operation.It was shown that there was a correlation between the level of anti-alpha-Crystallin antibody and IgG in aqueous fluid after phacoemusification.Conclusion:Aqueous flare and corneal edema is related to humoral immune responses after phacoemulsification, and humoral immune responses against the crystallin can only play a role in severe anterior segment inflammation.
