ObjectiveTo investigate the efficacy and safety of percutaneous nephrolithotomy combine with thefourth generation of EMS calculi clearance system in treatment of calculous pyonephrosis.MethodFrom2012.Aug to2013. Nov,17cases of calculous pyonephrosis were treated bypercutaneous nephrolithotomy(PCNL) in our department.12cases were treated byrenalpuncturefistulization and then by phase Ⅱ surgery.5cases were treated by phase I surgery.Low perfusion pressure were severely kept during the intraoperative period,and the operationtime remain within1hour.Result13cases urinary calculi were removed at one time,1case whose residual calculus>1.5cm,was treated by percutaneous nephrolithotomy through another thoroughfare after1week.And3cases, whose residual calculi <1.5cm, were cured by ESWL(extracorporeal shock-wavelithotripsy). The successful rate of lithotripsy was13/17.The results proved that this techniquecould Rapidly relief the obstruction, and remove the The renal pelvis abscess. ConclusionPercutaneous nephrolithotomy(PCNL) is an safe and effective method of treatingpyonephrosis combined with upper urinary tract calculi. This techunique can be divided intoprimary (PCNL) or II–stage-PCNL(percutaneous nephrolithotomyafte r renal fistula) treatment.
