The Expression And Clinical Significance Of GR And E-cad In Bladder Transitional Cell Carcinoma

Objective:To observe the expression of glucocorticoid receptor (GR) and epithelial cadherin (E-cad) in transitional cell carcinoma and normal transitional epithelium,and to elucidate the potential relationship between the protein expression of GR and E-cad and the clinicpathologic features (age,gender,tumor stage,histological grade and recurrence condition) of bladder transitional cell carcinoma,thus to evaluate the diagnostic and/or prognostic value of GR as a new tumor molecule marker in bladder transitional cell carcinoma,and to discuss the possible relationship between GR and E-cad in epithelial-to-mesenchymal transition of bladder transitional cell carcinoma.Methods:Immunohistochemical staining for GR and E-cad was Performed on Paraffin embedded sections from65bladder transitional cell carcinomas and10normal mueosa from Patients who had been treated with surgery in our hospital from2011to2013. In the bladder transitional cell carcinomas,45cases of male,and20cases of female; Aged44to86years old,average65years old; according to the clinical stage,27cases of superficial tumor,and38cases of invasive tumor (T222cases,T39cases,T47cases); according to pathologic stage,48cases of high grade,17cases of low grade;43cases of incipience,22cases of recurrence. The data are analysed by SPSS13.0.Result:1.9cases of normal transitional epithelium are positive in GR,and the positive rate is90.0%,and58cases of the specimen of bladder cancer are positive in bladder cancer, and the positive rate was89.2%, the two positive rates show no statistically difference. But the high expression rate of GR in normal mucosa is50.0%(5/10), which is significantly higher than20.0%in bladder cancer(13/65).In bladder cancer,7cases showed high expression in invasive bladder cancer group,and6cases are in superficial bladder cancer group,the difference was not statistically significant (P>0.05);9cases are in high Pathologic grade,and4cases are in low pathologic grade,thus the difference shows no statistical difference (P>0.05);Incipient group of4cases,and9cases of recurrence group,and the difference had no statistical difference(P>0.05);Age and gender groups had no statistical difference.2. The abnormal expression rate of E-cad is0%in all normal transitional epithelium,and30cases of the specimen of bladder cancer are abnormal in E-cad,and the abnormal rate was46.1%, thus the two rates show statistically difference. In bladder cancer,23cases are in invasive bladder cancer group,and7cases are in superficial bladder cancer group, thus the difference was statistically significant (P<0.05);28cases are in high Pathologic grade, and2cases are in low pathologic grade, thus the difference was statistically significant (P<0.05); Incipient group of16cases,and14cases of recurrence group, and the difference was statistically significant (P<0.05); Age and gender groups had no statistical difference.3. Analysis showed that expression of GR and E-cad in bladder cancer has no correlation (r=-0.075).Conclusion:1. GR expressed in both bladder transitional cell carcinoma and normal transitional epithelium, the expression rate had no statistical difference. The high expression rate in normal transitional epithelium is higher.And in bladder cancer,the high expression rate is not related with the clinicpathologic features (age,gender,tumor stage,histological grade and recurrence condition)2. Comparing with normal transitional epithelium,the abnormal expression rate of E-cad protein in bladder transitional cell carcinoma is higher,and is closely related to the tumor pathological grade and clinical stage:the higher pathological grade and the clinical stage,and the higher the abnormal expression rate of E-cad protein. Detection of bladder transitional cell carcinoma tissue of E-cad protein expression is helpful to the diagnosis of bladder cancer,also helpful to judgment of prognosis of bladder cancer.3. Statistics analysis showed that the expression of GR and E-cad in bladder transitional cell carcinoma is not relevant,and this may indicate that,although both the molecules participate in epithelial-mesenchymal transition of bladder transitional cell carcinoma,the steps and stages involved are different,so no direct link occurs.
