Objective:To evaluate the clinical significance of laparoscopic appendectomy in the treatment of complicated appendicitis. Methods:A retrospective analysis of the patients with appendicitis received treatment in our department, from 2010.01 to 2011.02, including 35 patients with laparoscopic appendectomy and 42 patients with open surgery. Compare the operative time, blood loss, incision length, postoperative antibiotic using time, intestinal function recovery time, postoperative time out of bed, wound infection rate, hospital stay, hospital costs between the two groups of patients.Results:Lparoscopic appendectomy operative time was (74.14±18.05) min and the open group (65.12±13.81) min, compared to a slight increase(p<0.05), but the laparoscopic appendectomy group was significantly better than open group in the other sides, including less blood loss(18.1±4.86mlvs22.3±4.31ml, p<0.05), less incision length(2.5±0.2cmvs3.4±0.6cm, p<0.05), less postoperative antibiotic using time(5.4±1.1dvs6.9±1.0d, p<0.05), faster intestinal function recovery(16.1±4.6hvs36.1±11.9h, p<0.05), fewer postoperative time out of bed(21.7±3.2hvs29.5±8.7h, p<0.05), less hospital stay(7.1±2.8d vs9.2±4.8d, p<0.05). Because of the acute inflammation, the two groups have the same wound infection rate (2.9%vs4.8%, p>0.05) and also the laparoscopic appendectomy has more hospital costs than the open surgery.Conclusion:Comparing with open surgery, laparoscopic appendectomy in the treatment of complicated appendicitis has its advantages, including fever trauma, less pain, smaller scars, rapid recovery, and fewer complications. It is an ideal surgical treatment of complicated appendicitis.
