Objective:Study the variation in the expression of IDO、FoxP3and CD105in CIN and cervical cancer, explore the relationship between IDO、FoxP3and CD105, revealing the action of IDO、FoxP3and CD105in cervical cancer progression and metastasis. Moreover, analyse pathological data and cervical cancer’s prognostic factors, then explore the relationship between IDO and the prognosis of cervical cancer, thereby provide a theoretical basis for diagnosis and immunotherapy of cervical cancer.Materials and methods:Collected138cases of cervical lesions’paraffin tissue samples and30cases of normal cervical from Tianjin Central Obstetrics and Gynecology hospital and divided into4groups:normal cervix、CINI、CINII-Ⅲ and cervical cancer, detected the expression of IDO、FoxP3and CD105by immunohistochemistry SP method. Followed-up the prognosis and combined with the general information and pathological for statistical analysis.Results:1.The difference in the expression between the groups(1)IDO is expressed mainly in the cytoplasm of tumor cells, sometimes in the nucleus and the membrane. The expression of IDO in cervical cancer (100%) is higher than CINII-III (86.1%) and CINI(25%), no expression in normal cervical tissue. The difference between groups is statistically significant, P<0.05.(2)FoxP3mainly expressed in the nucleus of tumor cells, sometimes in the cytoplasm. The expression of FoxP3in cervical cancer (94.3%) is higher than CINII-Ⅲ (50%),the difference has statistically significant,P<0.05;but there’s no significant difference between CINII-Ⅲ group and CINI group(31.2%), P>0.05.(3)CD105was expressed in the cytoplasm of epithelial cells of blood vessels. The expression rate of CD105in cervical cancer group>CINII-Ⅲ group>CINI group, The difference between this three groups has statistically significant,P<0.05.2. The correlation of these three factorsThe Spearman correlation analysis shows that IDO and FoxP3expression has positive correlation(r=0.686, P<0.05), and IDO and CD105has positive correlation.3.The relationship between IDO、FoxP3、CD105and the clinicopatholgical featuresThe expression of IDO and FoxP3in cervical cancer has relationship with the stage grade、lymphatic positivity and histological grade, but no relationship with the pathological type and vascular invasion. The expression of CD105in cervical cancer has relationship with the depth of invasion、stage grade、lymphatic and vascular metastasis, but has nothing to do with the pathological type and histological grade.4.The prognostic factors in patients with cervical lesions(1)Factors of recurrence in patients with cervical intraepithelial neoplasiaThe regresson rate of CINI patients was87.50%,the progression rate was3.13%.The patients who did not regress were older than35years(P>0.05),75%patients showed high-risk HPV infection(P>0.05),and all the patients who did not regress showed IDO expression(P<0.01).The residual lesions and relapse rate of CINII-Ⅲ patients has no relationship with age (35years old for the sector),but has the relationship with the high value of preoperative HC2and postoperative HPV infection. I n addition,the IDO positive patients’ residual lesions and relapse rate was higher than those of negative patients, but the difference was not statistically significant, P>0.05.(2) Analysis of prognostic factors of cervical cancerFollowed-up70cases of cervical cancer patients,lost6cases. The median survival time was60.5months,5-year OS was95.3%,5-year DFC was79.7%.The prognosis of cervical cancer has relationship with cervical cancer stage、type、lymph node metastasis, P<0.05,no relationship with the age, grade and vascular invasion, P>0.05.The level of IDO expression significantly associated with the prognosis of cervical cancer.Conculsion:1.The expression of IDO、FoxP3and CD105in cervical cancer was higher in CIN, suggesting that these three factors closely related with the development of cervical cancer.2. The expression of IDO was positively correlated with FoxP3and CD105, it suggests that impact of IDO on tumor immune microenvironment and thus leading to the development and metastasis of tumor is not only related with its mutual interaction with Tregs cells, but also is closely related to tumor angiogenesis.3. Prognosis analysis showed that the patients with CIN always relapsed within2years after treatment, and the impact factors include persistent HPV infection、margin status and immune environment (IDO expression). The prognosis of cervical cancer has relationship with cervical cancer stage、pathological type、depth of stromal invasion、lymphatic and vascular invasion and the expression of IDO. In addition, the expression of IDO is an independent factor for cervical cancer prognosis. This provided a strong basis for the comprehensive treatment of cervical cancer, especially immunotherapy.
