Objective To compare the average times and the incidence rate of the complication of endoscope-assisted and non endoscope-assisted ventricular-peritoneal(V-P) shunts and analysis the reason of the difference. Methods The records of190patients who had undergone V-P shunt endoscopically(n=86) and nonendoscopically(n=152) for hydrocephalus between January2007and January2009were retrospectively reviewed. There were no significant differences between the2group in gender, age, etiology, surgical approach and kinds of shunt pipe.Collect the Follow-up date, The independent effect of endoscopic versus non endoscopic shunt, the average times of the complication analyzed with t test, and the incidence rate of the complication analyzed with chi-square test. Results Follow-up1-12month, the average times of the complication of endoscope-assisted was6.41±2.61month, the average times of the complication of non endoscope-assisted was5.03±2.67month, There were significant differences between the2groups (t=2.1662P<0.05).The incidence rate of proximal obstruction of endoscope-assisted and non endoscope-assisted were23.3%and36.8%, There were significant differences between the2groups (x2=4.664P<0.05). The major incidence rate (infection, proximal obstruction, fluidify and haemorrhage) of endoscope-assisted and non endoscope-assisted were26.7%and42.8%, There were significant differences between the2groups (x2=6.048P<0.05)Conclusion Endoscope-assisted (V-P) shunts extend the average times of the complication and decreased the incidence rate of proximal obstruction and the total incidence rate of complication. Endoscope-assisted (V-P) shunts improved the overall shunt survival as compare with non endoscope-assisted (V-P) shunts.
