Objective:Endometrial polyps are common gynecological tumor like lesions, can occur at any age, more in the40-60years old.The incidence of reported in the literature for7.8%-34.9%. Compared with premenopausal endometrial polyps, the rate of postmenopausal endometrial polyps in premalignant and malignant transformation rate can be as high as4.1%. Because there are a lot of reasons induced endometrial polyps, the pathogenesis is not very clear. Generally believed that the disease has a close relationship with the inflammatory reaction, the stimulation of estrogen and progesterone, the level of estrogen receptor and progesterone receptor. This study is intended to observe the expression of ER, PR, MMP-9, Ki-67in endometrial polyps of the postmenopausal women, to explore the pathogenesis of uterine endometrial polyps, and find the associated protein which may predict endometrial polyp canceration after menopause, to guide the treatment and prognosis.Method:The specimens in the experiment were selected of in our hospital from January1,2013to December31,2013period from the operation, All the patients had no weight criteria for obesity, hypertension, diabetes history, not taking non steroidal anti-inflammatory drugs, tamoxifen, estrogen progestin replacement therapy with no experience. Included18cases for the operation removed the postmenopausal endometrium specimens,30cases for the hysteroscopic removal or transecervical resection endometrial polyps specimens of postmenopause uterine. The experiment was divided into two groups, postmenopausal endometrium group as the control group. The condition is usually healthy, postmenopausal over1years, total hysterectomy for uterine prolapse vaginal, atrophy of uterus in preoperative ultrasonography, endometrial thickness <5mm, Intraoperative sectional specimens, see intimal smooth, atrophy, pathologically confirmed normal postmenopausal endometrium.Postmenopausal endometrial polyps groups as the experimental group. The condition is usually healthy, postmenopausal over1years because of vaginal discharge, bleeding, abdominal pain, routine ultrasonography endometrial thickness of5mm, lack of uniform, further prompted that endometrial polyps by hysteroscopy, then hysteroscopic endometrial polyp removal or hysteroscopic endometrial polypectomy, pathologically confirmed endometrial polyp, and to exclude other endometrial lesions. Using immunohistochemical method, detect the expression of ER, PR, Ki67, MMP-9in the two group.. The semi quantitative scoring method, the average percentage of glands in the two group and the interstitial cells positive cells were evaluated.Result:The positive expression rate of ER in experimental group and control group were93%and78%,higher than that of the control group, ER protein strong positive expression in the experimental group; the positive expression rate of PR in experimental group and control group were87%and67%.PR protein in the experimental group is higher than that of the control group was strong positive expression; the experimental group and the control group in the expression of Ki-67positive rates were53%and33%, the positive expression of Ki-67was weak positive, and the positive expression in experimental group was higher than that in control group; the positive expression rate of MMP-9in experimental group and control group were33%and11%, the positive expression of MMP-9was weak positive, and the positive expression in experimental group was higher than control group.Conclusion:1). The increase of the ER, PR content can occour to the postmenopausal endometrial polyps;2) Expression of Ki-67, MMP-9participates the generation in postmenopausal endometrial polyps;3). The expression of Ki-67, MMP-9in postmenopausal expression endometrial polyps cannot be as a predictor of endometrial polyps to the development of the endometrial carcinoma.
