Objective: This issue mainly studied the relationship between efficacy ofantiviral treatment in chronic HBV infection of the Chinese Han ethnical populationand IL-28B gene polymorphism (SNPs rs12979860、 rs8099917、rs12980275、rs7248668),Compared serum IL–28B level of chronic hepatitis B patients withhealthy controls’, which would provide the basis for chronic HBV infection antiviraltreatment curative effect prediction,optimized treatments,cost saving and reduced theeconomic burden of patients。methods: A total of59cases was enrolled,according to the antiviral drugs whichwere divided into four groups,effective group of interferon treatment,invalid groupof Interferon treatment,group effective of Nucleoside analogues (acid) treatment andinvalid group of nucleoside analogues (acid) treatment。Collecting peripheral bloodsamples of all cases,and serum was isolated from blood samples.collecting age,gender, family history of all cases;the patients o f the cases group were routinelyinvestgated for HBV DNA level, baseline level of ALT, AST, ALB, TB, HBeAgand HBeAb;Using ELISA to detect serum IL–28B level;Using the SNaPshottechnology to detect IL–28B nucleotide polymorphisms (SNPS, rs12979860、rs8099917、rs12980275、rs7248668)。Choiced59cases of healthy blood donors as thecontrol groups, determined the serum level of IL-28B。All the datas used statisticalmethods to deal with。Results:1. baseline characteristics of Interferon group:Effective group of14cases(man/women:9/5),Invalid set of15cases (man/women:11/4).Cases in bothgroups of age, baseline-HBVDNA levels, ALT, AST,ALB, by T test analysis,nostatistical differences between groups(P>0.05).The constituent ratio of sex、familyhistory in the two groups was analysised byχ2test,no statistical differences between groups(P>0.05).The level of TB、HBeAb have statistically significant differencesbetween two groups by t test analysis(P <0.01),However, The level of HBeAg hasdifferences between groups,by t test analysis (P <0.05).2. IL–28B genepolymorphism analysis of Interferon treatment group:Two groups of cases weredetermined IL-28B nucleotide polymorphisms (SNPS, rs12979860、 rs8099917、rs12980275、rs7248668), For genotyping,four kinds only be founded C/C and C/T、T/T and T/G、A/A and A/G、G/G and G/A,does not found T/T、G/G、G/G andA/A.SNP rs12979860(CC/CT): Effective group VS invalid group=14/0VS14/1,SNP rs12980275(AA/AG):Effective group VS invalid group=14/0VS14/1,adoptingFisher’s Exact Test, p-value=1, no statistical differences.SNP rs8099917(TT/TG):Effective group VS invalid group=14/0VS10/5,SNP rs7248668(G/G'G/A):Effective group VS invalid group=14/0VS10/5, adopting Fisher’s ExactTest,p=0.04215,there are statistical difference between two groups.3. The baselinecharacteristics of nucleoside analogues (acid) group:Effective group of24cases(man/women:18/6),Invalid set of6cases(man/women:3/3).Cases in both groupsof age, baseline-HBV DNA levels, ALT, AST,ALB, TB,HBeAb,by t testanalysis,no statistical differences between groups(P>0.05).The constituent ratio ofsex、family history in the two groups was analysised by chi-square test,no statisticaldifferences between groups(P>0.05).The level of HBeAg has statisticallysignificant differences between groups,by T test analysis(P <0.01).4. IL–28Bgene polymorphism analysis of nucleoside analogues (acid) treatment group:Twogroups of cases were determined IL-28B nucleotide polymorphisms (SNPS,rs12979860、 rs8099917、 rs12980275、rs7248668),For genotyping,four kinds onlybe founded C/C and C/T、T/T and T/G、A/A and A/G、G/G and G/A,does not foundT/T、G/G、G/G and A/A.SNP rs12979860(CC/CT)、 rs12980275(AA/AG)、rs8099917(TT/TG)、 rs7248668(G/G and G/A) four site genotype distribution,effective VS invalid group are all23/1VS5/1,adopting Fisher’s Exact Test,there areno statistical difference between two groups,(P=0.3655).5. The serum IL-28B levelwas lowerin patients with chronic HBV infection than inhealthy subjects. Conclusions:1.The baseline levels of TB, HBeAb titer levels of Chronichepatitis b patients is meaningful to predict the efficacy of interferon trement,while ismeaningless to nucleoside analogues (acid) curative group。HBeAg titer levels ismeaningful to interferon Or nucleoside analogues (acid) treatment,the lower of thetiter levels,the better of the result。2. Genetic distributions of IL-28B SNPS(rs12979860, rs12980275) between the groups treated by interferon or nucleosideanalogues (acid) had no statistical difference,namely with interferon and nucleosideanalogues (acid) antiviral treatment the curative effect of prediction has no relevance.3. Genetic distributions of IL-28B SNPS ((rs8099917T/T and T/G, rs7248668G/Gand G/A)) has no statistical difference in nucleoside analogues (acid) groups,whilehas satistical difference in interferon groups, prompt curative effect of interferonpredict meaningful, including rs8099917T/T genotype, protective rs7248668in G/Gfor protective genes. Rs8099917in T/T for protective genotype, rs7248668in G/G forprotective genes.4. The serum IL-28B levels was lower in patients with chronicHBV infection than in healthy subjects.
