Objectives: As a clinical frequently-occurring disease, the number of patientswith diabetes has seen a global rapid growth in recent decades. With the improvementof social economy and living standards, together with people’s life style changes,diabetes is expected to increase rapidly in the future with the various complicationsdramatically growing. From the data released from world health organization (WHO):in1994the number of patients with the disease reached120million and it wasincreased by0.15million three years later. It is expected to reach more than300million by2030. The disease is within the scope of “Xiaoke” of TCM, and had beenmentioned in Huangdi Neijing early in the2ndcentury BC. This thesis attempts toexplore the relationship between dedifferentiated classification based on TCM andchronic complications of diabetes through collecting clinical cases, so as to providethe basis for future prevention and treatment of chronic diabetic complications.Materials and methods: Through the retrospective research approach,134cases of patients with diabetes (April2012-December2013)from the secondaffiliated hospital of Dalian medical university with qualified diagnostic standardwere selected. The relationship between diabetes and its chronic complications wereanalyzed in an attempt to explore the distribution characteristics of diabetes and itschronic complications. SPSS17.0statistical data for processing method was adoptedfor the statistical analysis of relevant data. Results:1. Among the134patients with diabetes,64patients are men,accounting for47.8%;70patients are women, accounting for52.2%. The ratio ofmale to female was1:1.09, therefore, there were no statistically significant differencebetween men and women.2. Of all the134patients with diabetes,113patients haddeveloped chronic complications and the distributions are: among the41cases withlung injury,31patients developed chronic complications, accounting for75.6%;among the34cases with “stomach heat flourishing”,26patients developed chroniccomplications, which accounted for76.5%; among the59cases with “kidney Yindeficiency syndrome”,56patients developed chronic complications have, accountingfor94.9%. Data from Table3was processed through statistical analysis andChi-square test was used between the three groups, it was found that P=0.011<0.05,which indicated statistical significance. Then every two groups were further comparedand it was ultimately concluded that P=0.03<0.05, which showed statisticalsignificance. Thus it can be concluded that patients with “kidney Yin deficiencysyndrome” are the most susceptible group to develop chronic complications.3.Among the113cases with chronic diabetic complications,70cases hadcardio-cerebral vascular disease,60cases had peripheral neuropathy,29cases hadretinopathy and8cases had diabetic nephropathy. Chi-square test was separately usedfor the three groups with complications, and it was found that P <0.05, which showedstatistical significance. Further comparisons were done between every two groups andfound that P <0.05, which showed statistical significance. Therefore, it can beconcluded that “kidney Yin deficiency syndrome” is responsible for all types ofdiabetic complications.Conclusion:1. Regarding TCM distribution of diabetes and its chronicdiabetic complications, priority is given to kidney Yin deficiency syndrome.2. The possibility of developing chronic complication with kidney Yindeficiency syndrome is higher than that from lung heat injury syndrome and stomachheat flourishing syndrome. Therefore, diabetes with kidney Yin deficiency is the mostvulnerable type to merge chronic complications (diabetic cardio-cerebrovasculardiseases, diabetic nephropathy, diabetic retinopathy, diabetic peripheral neuropathy). And this is in consistent with the important theory of traditional Chinese medicinewhich holds that Xiaoke is attributable to the viscera—the lungs, stomach and kidneys,and the kidneys play especially vital roles.3. Clinical prescription is based on the basic pathogenesis of TCM syndromedifferentiation, which is Yin deficiency as the root cause, hot and dry as themanifestation, so the treatment should focus on keeping Yin fluid, eliminating heatand moisturizing dryness; the treatment for lung injury focus more on moisteninglungs, together with nourishing kidney Yin; priority is given to stomach heat treatmentfor stomach heat flourishing, with nourishing kidney Yin as the supplementary means;Kidney Yin deficiency focuses on nourishing kidney Yin.
