Objective:To analyse the level of100cases of thyroid nodule surgery patients’sBMI, blood uric acid, sTSH, FT3, FT4, Anti-TG, Anti-TPO, fasting glucose, fastinginsulin, insulin resistance index, To compare the difference between benign andmalignant thyroid nodules, and further explore the relationship between the benign ormalignant nodules and insulin resistance.Methods:100patients from General Surgery of the Second Affiliated Hospital ofDalian Medical University during June13,2013-August19,2013,were taken blood fordetecting blood uric acid, sTSH, FT3, FT4, Anti-TG, Anti-TPO, fasting glucose index.To collect their fasting serum before operation, and place at-20℃storage. Fastinginsulin was measured in August201328, in our hospital laboratory.Results:1.100cases of thyroid surgery in patients with benign nodules49cases,accounting for49%, including39females, accounting for79.6%, male10cases,accounting for20.4%; malignant nodules group of51cases, accounting for51%, ofwhich34cases of women (66.7%),17males (33.3%), including50cases ofpapillary thyroid carcinoma, one case of medullary thyroid cancer, the latter notincluded in the statistical analysis. The average age of benign nodules (49.40±11.10) years old, the average age of malignant nodules group (46.68±12.72) years of age.Two groups of male and female prevalence was no significant difference (P>0.05).2. The fasting insulin, insulin resistance index of malignant thyroid nodules’ groupwere slightly higher than the benign nodules’ group, but the two groups showed nosignificant difference (P>0.05). Compare two groups’ BMI, blood uric acid, sTSH, FT3,FT4, Anti-TG, Anti-TPO, fasting blood glucose,there were not statistically different(P>0.05).3.Benign and malignant thyroid nodules groups stratified according to insulinresistance index:<1.0group contains23cases of malignant nodules and27cases ofbenign nodules, the ratio of the number of cases of malignant/benign was0.85,1.0~2.0group contains19cases of malignant nodules and19cases of benign nodules, theratio of the number of cases of malignant/benign was1.0,>2.0group contains8cases of malignant nodules and3cases of benign nodules, the ratio of the number ofcases of malignant/benign was2.67. With increased insulin resistance, the proportionof malignant thyroid nodules gradually increased, but there was no significantdifference (P>0.05) between the layers of the two groups of patients.4. Logistic regression analysis of groups of benign and malignant nodules showedthat age, sex, BMI, sTSH, anti-TG, fasting glucose, fasting insulin were negativelycorrelated with thyroid cancer, FT3, FT4, anti-TPO, insulin resistance index, serumuric acid were positively correlated with thyroid cancer, but the Wald chi-square testP>0.05, not statistically significant.Conclusion:1.The levels of the fasting insulin and insulin resistance index in thyroid malignantnodules group are slightly higher than that of the benign nodules group, but nosignificant difference between two groups. 2.With insulin resistance index increasing, the proportion of malignant thyroidnodules gradually increase, but there is no significant difference between two groups.3.There is no statistically difference in age, sex, BMI, sTSH, FT3, FT4, Anti-TG,Anti-TPO, uric acid, fasting blood glucose parameters between malignant and benignnodules groups.4.There is no correlation in age, sex, BMI, sTSH, FT3, FT4, Anti-TG, Anti-TPO,uric acid, fasting glucose, fasting insulin, insulin resistance index between malignantand benign nodules groups through multivariate logistic regression analysis.
