ObjectiveResearch the influence of ultrastructure changes in DEJ after scar treatment with fractionalCO2laser, and compare the influences of these two wound repairing methods to theultrastructure in DEJ zone.MethodsWe make1.0cm*1.0cm Full-thickness skin defect wound on the rabbit’s ear,160woundstogether for each ear has5wounds. By changing the dressing every other day in60days, thescars are thickened, softened and smoothened but also fading. The experiment is divided intofour groups: normal skin group, scar group, scar laser treatment moist exposed group (moistexposed group) and scar laser treatment iodine group (iodine group)。After the fractional CO2laser treatment, we apply moist exposed group to the wound in themoist exposed group and iodine in the iodine group once every3-4hours,but nothing to thenormal skin group and the scar group。Then we observe the wound healing and record thehealing time.We collect the tissue by normal skin group、scar groupand do biopsy1W,2W,4W and8Wafter treatment, using light microscopy observed nipple, skin nails, basement membranemorphology in HE staining, PAS staining and electron microscopy observed DEJ zoneUltrastructure changes.ResultsGeneral observation:scar group:the wound pink, smooth surface and soft touch of qualityand slightly out of the plane.Iodine group: The wound becomes dry and scabby in2-3daysafter the treatment; the crusts fall off in9-10days; wound healing in14days. The scars aresmoothened and fading.Moist exposed group: The wound moist after treatment,the woundsscattered Point-like scabs in2-3days, the scab fall off the scar in5-6days.The iodine group’swound healing time is (13.6±0.52)days,and the moist exposed groups is (10.9±0.74)days.With a significant difference between the groups has an evident statistical significance(P<0.01)。Histopathological Observation: normal skin group: the basement membrane is thickness,uniform, continuous and serrated, the hemidesmosomes was like buttons, anchorfiber dense arranged in neat rows. Desmosomes between two adjacent epithelialcells,connection with Porphyritic shape, tension fibril bundles tightly packed, uniformdensity.scar group:the first1W basement membrane rupture is not continuous andthickness,the number of hemidesmosomes(0.44±0.08)and desmosomes was reduce,anchorfiber(0.28±0.09)and tension tow density decreased and disorganized。In the2W、4W,the number of hemidesmosomes(2W:0.64±0.17,4W:0.92±0.23)and desmosomes isincrease,anchor fiber (2W:0.64±0.17,4W:0.92±0.23) and tension tow fiber densityincreases (P <0.05).Compared with the4W,in the8W the basement membrane rupture, thenumber of desmosomes, hemidesmosomes(1.08±0.11) and anchor fibers(0.96±0.08)and fiber bundles tension wire density did not change significantly (P>0.05). After thefractional CO2laser treatment: the2ndW,appeared the newborn basement membrane anddepression newborn anchor fibers(1.04±0.07)in moist exposed group; the4thW, thenumber of desmosomes, hemidesmosomes(2.08±0.27) obviously increased, anchor fiber(1.52±0.22) density increased, the continuity of the basement membrane was obviouslyincreased,Until the8thW, the nipple shape and the number of hemidesmosomes(2.96±0.26)and anchor fibers(2.04±0.33) close to the normal skin(hemidesmosomes:3.44±0.26;anchor fibers:2.52±0.23)、the basement membrane thickness uniform, continuous.Conversely, in the2ndW, iodine group wounds basement discontinuity, reducing the numberof desmosomes and hemidesmosomes(0.96±0.08)、anchor fibe(r0.84±0.10)structuralfeatures not obvious,in the4thW, the basement become more continuous.in the8thW,appeared that the flat nipples, basement membrane is not continuous and number ofhemidesmosomes(2.12±0.30)and anchoring fibers(1.68±0.12)was significantly lessthan the moist exposed group (P <0.05).Conclusion1. Fractional CO2laser could lead to scar DEJ zone ultrastructure changes.2.A moist environment for the wound healing is conductive to the DEJ zone ultrastructurereconstruction...
