Coronary Artery Characteristics Comparison Of Acute Non-ST-Elevation Myocardial Infarction Patients With Or Without ST-segment Depression

Background and objectiveAs is known to all, acute myocardial infarction (AMI) were divided into ST segment elevation acutemyocardial infarction (STEMI) and non ST segment elevation acute myocardial infarction (NSTEMI)according to ST segment elevation. ECG showed ST depression and no offset of NSTEMI is objectiveexistence. We consulted the documents, but no further discoveries. The study observed the features ofcoronary artery lesions of NSTEMI patients which ECG ST segment depression or not, to understand thesimilarities and differences between the two groups.MethodsA total of74NSTEMI patients who were diagnosed and received percutaneous coronary interventionfrom July2008to June2013were included in the present study. We recorded the baseline data andcoronary angiography. There were55males (74.47%),19cases of female patients (25.53%) of thesepatients, age36-86(60.85±12.35years), ST-segment depression group44(59.46%), ST-segment withoutdeviation group30(40.54%). Based on presentation≤72h from symptom onset to emergency coronaryangiography whether ECG appeared ST-segment depression, NSTEMI cases were classified as ST-segmentdepression group or ST-segment without deviation group. Determination results of coronary angiography isjudged by two interventional cardiologist. Diagnostic criteria for acute myocardial infarction(AMI)infarction related artery(IRA):1.the thrombus of visible image;2.visible patches formed off ulcer;3.Tocompletely block or the narrow place as IRA. All vessels are divided into near period, middle and far.Among them, the left anterior descending artery(LAD) was divided into near period, middle and far; leftcircumflex artery (LCX) was divided into the near and far; right coronary artery(RCA) was divided intothree sections. Recording the indicators for lesions of coronary artery,including position, numbers,collateral circulation, location, degree of stenosis, etc. Analysis of the two groups of clinical baseline data、emergency coronary angiography results and ECG’s dynamic evolution; SAS v9.1software was used forstatic analysis. All the Measurement data in this study are expressed in the mean±standard deviation (x±s),the measurement data of the two groups of were compared using independent samples t test. Enumeration data were compared using chi-square test, the exact probabilities are used when the theoretical frequency isless than1or the total number of samples is less than40. The correlation are tested using Pearson linearanalysis. All statistics were two-sided test, a P value of less than0.05(2-tailed) was statistically significant.Results1. In terms of risk factors for the two groups, hypertension, high cholesterol, diabetes, smoking history,history of coronary heart disease (CHD) were analyzed, p>0.05, no statistical significance between the twogroups.2. In this study, compared two groups of number of diseased vessels, single branch lesion of P<0.05,was statistical significance. The single branch lesions of NSTEMI is high. Compared two groups of lesions,complete occlusion of P<0.05, collateral circulation of P<0.05, was statistical significance. There were12cases of single branch occlusion,5cases of LAD distal lesions and7cases of LCX distal lesions. Therewere7cases of LAD distal lesions and LCX distal lesions, class TIMI1. There were19cases of collateralcirculation of no drift group.3. In this study, there were40cases of a plurality of non complete occlusion lesion of ST segmentdepression group and the two groups of patients both had a higher incidence of multivessel disease.Conclusions1. The coronary artery disease of ST-segment depression in NSTEMI patients mostly had multi-vesselnon-occlusive lesions.2. The coronary artery disease of non ST-segment deviation in NSTEMI patients was the single-vesseltotally occlusion or single-vessel incomplete occlusion, grade TIMI1or had abundant collateral circulation.3. The industry has proposed NSTEMI into ST-segment depression and ST-segment depressiongroups.
