| Background and ObjeciveThe treatment of acute lower limb ischemia has changed a lot to improve theprognosis at present.This article aim to assess the influence on prognosis ofintraoperative intra-arterial infusion of urokinse during hybrid operation on acutelower limb ischemia.MethodsRetrospectively study the data of163patients with acute lower limb ischemiasuffered hybrid operation between August2009and October2012.78cases got onlysurgery treatment,86cases received intraoperative injection of urokinase. Comparedand analyzed the clinical outcomes of the two groups.ResultsAfter operation, the ABI of Blank group and urokinse group average increased0.69±0.17and0.74±0.14respectively. The recovery ratio of posterior tibial artery ordorsal pedal artery was76.34%and87.76%. The difference of these have statisticalsignificance. One month and18months after surgery, the mortality rate of blankgroup and urokinase group was3.85%vs3.53%and11.51%vs12.94%respectively. The amputation rate was12.90%vs9.18%and22.58%vs11.22%respectively. Thereocclusion rate was16.13%vs6.12%and26.88%vs14.29%respectively.Thereoperation rate was3.85%vs2.35%and14.81%vs7.61%.The average ABI was0.86±0.16vs0.90±0.13and0.76±0.10vs0.82±0.14respectively.The reocclusion rateand ABI of both time have statistical significance. The amputation rate of18monthshave statistical significance. Amputations related risk factors analysis showed thatpatients with diabetes and atrial fibrillation, thrombosis and occlusion length is morethan10cm and ischemia belongs to Ⅱb have high risk.ConclusionsThe hybrid operation has a good curative effect on treatment of acute lower limbischemia. Intraoperative intra-arterial urokinse infusion for patients with acute lowerlimb ischemia under hybrids operation may contribute to improve the rate of arterypatency and prognosis. |
