With the rapid development of modern medical technology, the fracture does notheal after surgery was significantly lower than the original, according to statistics,about5%-10%of the fractures can occur for various reasons nonunion. Nonunionfracture healing is a common complication in a clinically refers fracture under certainconditions affect fracture healing function stops, the fracture has formed a false joint,mainly for the fracture during physical activity significantly abnormal activities, andpain is not obvious disease. Treatment of femoral shaft fractures, intramedullaryinterlocking nail technology has matured, with its axis intramedullary fixation, fix-ation and difficult to break and so gradually replace the steel femoral shaft fracturesbecome mainstream. The backbone of the interlocking intramedullary nail for fem-oral fractures in clinical internal fixation is ideal because of its advantages are: theability to maintain good anatomic or near anatomic reduction, fixation, can be earlyambulation and functional exercise. Interlocking intramedullary nail so widely used inclinical nonunion decreased, but sometimes because of the patient’s own conditions orother factors that can lead to many nonunion after intramedullary nail, is a seriouscomplication of late fractures. Reported after intramedullary nail incidence of nonuni-on1.8%to7%, while the femoral shaft fracture nonunion after intramedullary inter-locking nail incidence rate of approximately1%. However, after the occurrence of thedisease in patients with pain and psychosocial withstand the dual pressures of treat-ment is quite tricky. So more and more concerned about their treatment and physicianachievements。Objective:In this study, through the analysis of long bone intramedullary nail postoperativenonunion factors observed using intramedullary nail to keep the original plus a singleLCP with a sufficient amount of cortical bone and remove the nail topical dual LCPwith a sufficient amount of cortical bone two kinds the clinical efficacy of surgical treatment of long bone intramedullary nail postoperative nonunion, comprehensive andobjective evaluation of the effectiveness and feasibility of using reserves plus the origi-nal single nail LCP with a sufficient amount of cortical bone graft surgical method,and worth clinical.Methods:Extracting the last two years between Orthopaedic Hospital of Zhengzhou Cityafter36cases of long bone intramedullary nail nonunion cases. And in the hospitalusing two different surgical treatments: the treatment group,18were treated with intra-medullary nail to keep the original plus a single LCP with a sufficient amount of corti-cal bone graft treatment; the control group,18patients using topical nail removedwith foot double cortex LCP amount of bone graft treatment. The clinical observationof the two groups were compared indicators such as blood loss, bed time, after1month,3months,6months,1year of imaging data (subject to extension if necessary) to obse-rve the healing time and their own records hip, knee and ankle function after surgery.Statistical analysis of the clinical efficacy studies using indicators to compare the twogroups to assess the two surgical methods.Results:In this study,36cases were obtained effectively followed up for6to15months,with an average9.96±1.21months, the treatment group had no nail, plate fracture, nonail and locking screw breakage bending, loosening, fracture were bone healing, thehealing rate of100%; the control group,2cases of broken plate, one case of postoper-ative infection occurs due to nonunion,13cases of bone fracture healing, two cases ofdelayed union, three cases of infection due to plate fracture and that there is no healing,the healing rate of83.3%. Two groups of blood loss, bed time, healing time, the rate offracture healing and limb and joint function were statistically analyzed P values wereless than0.05, the difference was significant proven clinical efficacy in the treatmentgroup than the control group.Conclusion:Interlocking intramedullary nail to keep the original plus one LCP with a suff-icient amount of cortical bone can be effective in the treatment of nonunion afterintramedullary nail, and body weight after surgery can be an early functional exercise,the surgical approach is safe and reliable, the healing rate, concurrent disease less,simple and effective, satisfactory results worthy of promotion.
