Comparison The Effects Of Miccoli Thyroidectomy Versus Conventional Thyroidectomy:a Meta-Analysis

Objective The purpose of this study was to evaluation the safety and efficacy of Miccoli thyroidectomy compared to open conventional thyroidectomy (CT) for thyroid benign nodular,and provide a more advantegous and rational treatment measures for clinicist appling thyroidectomy.Methods Making strict inclusion criteria and exclusion firstly,Literature search were performed by computer on all randomized controlled trails reported on Miccoli thyroidectomy versus open conventional thyroidectomy for thyroid benign nodular between January2000and July2013from datas such as CBMdisc,CNKI,WangFang data,VIP data,PubMed,OVID and MEDLINE data.Date analysis was performed by RevMan4.2,as long as compare these the length of incision,the operative time, intraoperative blood loss,postoperative complications, hospital-lization time and the expenses of hospitalization.Results After filtering,there are17randomized controlled trials were included,13Chinese and4English.Using meta solftware weighted mean difference (WMD) and the confidence interval (CI).The results of meta—anlaysis indicated that in comparison with open conventional thyroidectomy,the length of incision (WMD=-4.83,95%,CI[-5.95,-4.06],Z=12.37,P<0.00001),intraoperative blood loss (WMD=-18.81,95%,CI[-25.90,-11.73],Z=5.20,P<0.00001),hospitalization time (WMD=-1.44,95%CI[-1.98,-0.90],Z=5.25, P<0.00001) of Miccoli thyroidectomy have advantages.While the operative time (WMD=17.77,95%,CI[9.35,26.18],Z=4.14, P<0.0001) of Miccoli thyroidectomy is longer than open conventional thyroidectomy and the expenses of hospitalization (WMD=2349.82,95%,CI[1532.11,3167.54], Z=5.63,P<0.00001) of Miccoli thyroid-ectomy group was significantly more than open conventional thyroidectomy group.There was no significant in postoperative complications (OR=0.89,95%, CI[0.55,1.43], Z=0.49, P=0.63)Conclusion Miccoli thyroidectomy can reduse the length of insicion, improve the cosmetic result,reduce the intraoperative blood loss and hospitalization time.The operative time of Miccoli thyroidectomy group is longer than the open conventional thyroidectomy group and the expenses of hospitalization also higher than the open conventional thyroidectomy group.There is no significant in postoperative complications between the Miccoli thyroidectomy group and the open conventional thyroidectomy group.
