Objective: To compare the differences between Miccoli 's endoscope-assisted thyroidectomy and conventional open thyroidectomy,as well as explore the security and efficacy of endoscopic surgeryMethods: From May 2010 to March 2011,56 patients with thyroid nodules underwent thyroidectomy in the General Surgery 2 of the first affiliated hospital of Dalian medical university .All the patients had single or multiple thyroid nodules,no malignant tumor performance and no Neck surgery and Radiotherapy.Before the surgery ,we explain to them the charicteristics and postoperative complications of the surgery.According to the patients'choice,we divided them into two groups: Miccoli group(26 cases) and open group(28 cases).Operation time,intraoperative blood loss,incision length ,hospital stay,serious complications and satisfaction with cosmetic results were compared.Results: Total , secondarily total or subtotal thyroidectomy was performed in all of the patients.No patient in the Miccoli group was converted to open surgery during the operation.In the Miccoli group(26 cases): operation time : (105.4±28.7)min;intraoperative blood loss:(22.4±11.6)ml;incision length:(2.0±0.3)cm; hospital stay(2.2±0.5)days;temporary injury of recurrent laryngeal nerve:1 case;injury of parathyroid hormone:0 case;satisfaction with cosmetic results:(9.3±0.4)points.In the open group(28 cases): operation time (98.1±22.4)min,intraoperative blood loss(47.3±18.2)ml,incision length(5.7±0.9),hospital stay(2.3±0.6)days,temporary injury of recurrent laryngeal nerve : 1 case;injury of parathyroid hormone : 0 case;satisfaction with cosmetic results:(4.4±0.7)points.Two groups of patients in the non-research factors such as age, sex ratio,size of the nodules was no significant difference (P> 0.05). Miccoli group had a longer Operation time (P<0.05),shorter incision length(P<0.05),less intraoperative blood loss(P<0.05)and better satisfaction with cosmetic results(P<0.05).Conclusions: Miccoli thyroidectomy are feasible and safe.Compared with conventional open surgery,Miccoli thyroidectomy has minor injure ,less intraoperative blood loss and better satisfaction with cosmetic results.Because of the similar operation of the open surgery,it requires low to the surgeon .Miccoli thyroidectomy is innovative and progressive of the thyroidectomy and it should be further promoted .
