Objective:Analyzed retrospectively summarized the clinical characteristics of orthopedics of knee pain after total knee arthroplasty patients of Guangdong Province Traditional Chinese Medical Hospital, to explore the causes of knee pain after total knee arthroplasty, specific risk factors, provide a reference for clinical prevention and treatment of pain after knee replacement, improve operation success rate and improve patient satisfaction.Methods:This study used a retrospective analysis of research methods, collect in June2010to June2013in Guangdong hospital of the clinical data of patients with total knee arthroplasty, select standard of postoperative patients with knee joint pain, by unit or outpatient follow-up, according to the domestic and foreign summary and analysis of the causes of the pain after total knee arthroplasty, common diagnosis method of TKA cases of postoperative knee pain to carry on the system analysis, summed up the reasons and characteristics of TKA postoperative pain. Count data using x2test, measurement data using independent t test sample wood, characteristics and reasons of TKA post-operative pain using descriptive analysis, all the data using SPSS19.0statistical software analysis, P<0.05as the standard was statistically significant.Results:Of this study were collected from primary osteoarthrosis, traumatic arthritis, rheumatoid arthritis, ankylosing spondylitis and other non suppurative arthritis and in patients undergoing primary unilateral total knee replacement treatment684cases,258cases of male patients (37.7%),426cases of female patients (62.3%). Which age<55patients with20cases (2.9%), aged55to75patients with576cases (84.2%), age>75-year-old patient88cases (12.9%). Disease types:primary osteoarthrosis486cases (71.1%),124cases (18.1%), rheumatoid arthritis, traumatic arthritis,48cases (7.0%), ankylosing spondylitis,6cases (0.9%), ischemic osteonecrosis in8cases (1.2%), the other12cases (1.7%).Found during the follow-up period appeared56patients postoperative knee joint pain, the pain incidence was8.2%, male patients,24cases (42.9%),32cases of female patients (57.1%), the incidence of male patients pain was9.3%, the incidence of female patients pain was7.5%. Which age<55group,3cases (5.4%) in pain, the incidence of15%;Group of patients aged55to75years old,46cases (82.1%) pain occurs, the incidence of8.0%;Age>75patients,7cases (12.5%) in pain, the incidence of8.0%. Through comprehensive analysis, the specific reason for the pain occurred mainly includes:(1) infection in6cases (10.7%);(2) joint instability in8cases (14.3%).(3) joint adverse to the line6cases (10.7%).(4) the knee adhesion,3cases (5.4%);(5) the abnormal joints line4cases (7.1%);Abnormal device (6) and10cases (17.8%);(7) prosthesis loosening8cases (14.3%).(8) around the prosthesis fracture3cases (5.4%);(9) the lumbar spine lesions in3patients (5.4%);(10) hip disorders in1case (1.8%).(11) the other4cases (7.1%).Conclusion:In this study, we through the review of the clinical data of684patients with total knee arthroplasty, the comprehensive analysis of56cases of TKA different reasons of postoperative knee pain, can draw the following conclusions:(1)different gender and age group patients TKA there was no statistically significant difference comparing with the incidence of postoperative pain, cannot think TKA the occurrence of postoperative pain associated with gender and age;(2)after TKA knee pain complex and varied reasons, diagnosis is difficult, a comprehensive analysis was carried out on the postoperative pain, misdiagnosis and can effectively avoid the misdiagnosis;(3) the TKA reason, postoperative pain is given priority to with intra-articular factor, accounting for about85.7%, including joint instability, prosthesis loosening and stretching knee device problem, the most common extra-articular factors accounted for14.3%, of which the majority with lumbar spine and hip lesions;(4)in knee pain before the main reason is the problem and device, and one of the most common is patellar instability.
