ObjectiveThe aim of this study was to observe the quantitative degree of tibia forward and the knee joint function-wise with the knee preoperative of anterior cruciate ligament reconstruction and postoperative follow-up. To explore the significance of the quantitative degree of tibia forward in evaluating the stability of the knee after ACL ruptured and reconstruction. Propose the optimum quantitative degree of tibia forward to diagnose the ACL rupture base on collecting and statistical the data. Providing an objective and quantitative index for the diagnosis of knee joint ACL injury and the directly compare of the knee stability change after ACL reconstruction and postoperative fallow-up.Methods58patients who underwent arthroscopic ACL reconstruction from4th Or-thopedics of No1. affiliate hospital of Guang Zhou University of Chinese Medicine from January2012to March2014were investigated retrospectivel y,55of whom were included in this study followed up by outpatient clini c. Male31cases, female24cases; Age:19-46years old, average30.8yea rs old. Causes:sports injury in46cases,5cases by traffic accidents, industrial injury in4cases. Damage (arthroscopic confirmed):the anter ior cruciate ligament pure rupture in30cases; ACL rupture merger menis-cus injury in21cases; ACL rupture with meniscus and medial collateral ligament injury in4cases. All cases with lumbar anesthesia, anterior c ruciate ligament reconstruction by arthroscopic, all patients surgery per-formed by the same group of doctors, take consistent postoperative rehab ilitation program。All patients in preoperative application with Kneelax3joint meter (the Netherlands, MONITORED REHABSYSTEMSB. V. models:Kneelax3) measure the degree of tibia forward. On these cases follow-up after the ACL reconstruction on a regular basis (after3months,6months,9month s,12months), each follow-up Kneelax3should be used in measuring instru ment to evaluate the stability of the Knee and record the Lysholm knee function score, and analyst the data to statistical and comparison.ResultsWith30°of knee flexion using Kneelax3joint tests measuring instrument, preoperative testing results are as follows:Pull with44N, the average of trouble knee forward (3.35±1.90) mm, the average D-value of couple knees forward (0.64±0.13) mm. Pull with66N, the average of trouble knee forward (4.73±1.70) mm, the average D-value of couple knees forward (0.88±0.12) mm. Pull with88N, the average of trouble knee forward (6.36+1.21) mm, the average D-value of couple knees forward (1.57±0.22) mm. Pull with132N, the average of trouble knee forward (8.63±2.32) mm, the average D-value of couple knees forward (2.51±0.24) mm. In132N pull, the95%degree of trouble knee tibia forward>8.91mm; the95%index degree of trouble knee tibia forward>1.63mm; the95%D-value of couple knees forward>2.52mm; the95%index D-value of couple knees forward>0.68mm.55patients received postoperative follow-up for12-18months. All the patients with postoperative knee stability more improved than preoperative (P<0.05), but as time pass, postoperative knee stability has certain decrease trend. The Lysholm scrod by preoperative increased from63.58±3.61scores to93.26±3.48scores, with significant difference (P<0.05).ConclusionKneelax3joint measuring instrument can be used for quantitative evaluation of knee ACL ruptures of the knee joint forward stability, the knee joint forward stability of the knee with ACL rupture is worse than the healthy knee. Kneelax3knee measurement can be used as an auxiliary means in the diagnosis and curative effect evaluation of knee anterior cruciate ligament injury. We obtain the reference value of diagnosis is in132N pull,95%the degree of trouble knee tibia forward>8.91mm;95%the V-value of couple knees forward>2.52mm. ACL reconstruction under arthroscopy can improve the stability of the knee, increase the Lysholm knee functional score, improve the quality of life of the patients. As time goes by, the postoperative knee stability has certain decrease trend.
