The Clinical Value Of MRCP Examination For Preoperative Cholecystectomy

[Objectiv]Through retrospective studing the detection rate of MRCP examination for choledocholithiasis before the cholecystectomy to discuss the value of MRCP examination for choledocholithiasis and provide reference for the rational use of MRCP examination.[Method]There are1655patients who were shoued with gallbladder stones by B ultrasonic examination from the second affiliated hospital of kunming medical university since January2009to December2013,among which659were male,female996cases,58.25±10.75years of age.All patients were divided into choledochlithiasis suspicious groups (230) and simple set of gallstones (1425), while comparing two sets of MRCP examination on detection rates of choledochlithiasis cholecystectomy and preoperative.248patients with choledocholithiasis according to the size of stone were divided into:choledocholithiasis calculi diameter d>1.0cm group,0.5cm<d<1.0cm group and d<0.5cm group, compared the diagnostic accordance rates of opreoperative MRCP for different size of common bile duct.All the comparison between the experimental group used chi-square test, P<0.05, there was statistical significances.The using of SPSS17.0software for statistical analysised. [Result]The detection rates of the preoperative MRCP examination for common bile duct stones was80%in the group of bravery manager suspicious,and the prevalence of common bile duct calculi was85.2%, the sensitivity, specific degree of MRCP were93.5%,94.1%respectively.The detection rate was3.4%in the Simple gallbladder stone group,and the prevalence of common bile duct calculi was3.7%, the sensitivity, specific degree of MRCP were92.3%,99.9%respectively.Two groups of line detection chi-square test, P<0.05, with statistical significance.Grouped by stone size: bravery manager diameter d>1.0cm group, preoperative MRCP examination diagnosis coincidence rate is87.1%;0.5cm<d<1.0cm group, the diagnosis coincidence rate is81.1%; d<0.5cm group, the diagnostic coincidence rate was57.8%.Bravery manager diameter d<0.5cm group respectively with d>1.0cm,0.5cm<d<1.0cm line set of chi-square test, P<0.05, there were statistical significance.[Conclusion]1.The detection rates of MRCP to check choledocholithiasis in Patients with common bile duct calculi suspicious are significantly higher than patients with only pure gallstones.2.There are great clinical value of MRCP examination for patients with choledocholithiasis suspicious.3.MRCP examination should not be a routine inspection in patients with pure gallbladder stone to save medical resources and reduce the patients’medical costs.4.There is low detection rate of MRCP examination in patients with tiny bravery manager stones.
