In recent years, as the conception of hospital management and the increases of patients’awareness of personal rights, countries around the world attach great importances to patient safety issues. Unplanned reoperation as one of surgical adverse events, increased the risk of postoperative organ dysfunction in patients, and directly affects the body to be recovered, eventually, lead to death. But through epidemiological methods, compositions and discussing ways domestic and foreign scholars make their own points about unplanned reoperation of foreign scholars’ research on pathological factors of analysis, these studies focus on comprehensive perioperative risks and risk management of the whole process of identification, quantitative analysis of synergy of multiple factors and its significance, but slightly, Therefore, it is very signigicant, how to identify risks, analyze risks and risk factors, effective evaluation of tolerable risk level, risk index system for perfecting surgical system, in order to develop a new risk management strategies, as well as the design of risk coping mechanisms.Based on unplanned reoperation risk assessment as the starting point, this paper apply the strong logical fault tree analysis method, which has the characteristics of high flexibility, wide application range, qualitative and quantitative combination, contains three steps:risk identification, risk analysis risk and evaluation analysis.First of all, for realizing the scientific and comprehensive recognition system risk of the operation, we synthetically use the literature search and expert consultation to collect the risk factors, including the pathologic examination, brainstorming joint method of data collection, which is to identify preoperative preparation, intraoperative and postoperative care operation and management throughout the whole process of perioperative, which might have relationship with the planned reoperation of inherent or potential risk source and risk factors. This method is adapted to the varied and complex surgical treatment system, the changing environment, risk performances and the characteristics of many risk influence factors, improving the reliability of risk identification and risk identification efficiency, being better for the surgical treatment system risk analysis and assessment services.Secondly, we have build up the construction of fault tree model for scientific analysis of surgical treatment systemic risk. Based on fault tree theory of composition, according to the logical relationships between each other events, we rearrang the surgical risks from thick to thin ranks, from small to big, hierarchically. Eventually, unplanned reoperation established fault tree, which is good for clear presentation of risks of surgical treatment system and the logical relationship between them. We made a qualitative analysis of fault tree in order to strike a minimal cut sets being clear for getting the risk aggregation of system failure, which lead to surgical treatment, find the possible failure and analyze the benefits. By the fault tree quantitative analysis, we gaind top event probability, probability theory based on risk factors of unplanned reoperation for likelihood of occurrence size forecast for the surgical treatment of systemic risk. This approach makes it clear to show the failure clauses of surgical system failure and possibility of various patterns, to explore the best ways to prevent risks, and improve system reliability and security, which provide the theoretical basis of surgical treatment, meanwhile, highlighting weaknesses which are good for improvement and control.Finally, we use the risk matrix to determine the risk levels, in order to preferably evaluate the risks of the surgical treatment system. Based on basic event probability of occurrence and severity of risk matrix mapping, according to the risk score, we determine the risk factors affecting the risk levels to assess the extent of their impacts on unplanned reoperation, after that we focused on the surgical treatment of risk management to ensure the effective use of health care resources. The risk evaluation of the surgical treatment method is applied to the system, by assessing risk level, risk factors of importance, provides support for diagnosis and treatment decisions.This article research results have great reference values for the risk control of surgical treatment system, and provide a research idea for the quality analysis and risk assessment of health care industry.
