objective: To investigate causes of death of Acute coronary syndrome(ACS) patients perioperative of percutaneous coronary intervention.methods:Collect40dead patients perioperative of PCI out of5388cases undergoing PCI treatment from January1,2010to June30,2013inDalian Central Hospital, to study and analyze the cause of death.Results:40cases of perioperative death ACS patients,20males and20females. Aged42to92years (mean69.78±11.71),32cases of AMIunderwent emergency PCI,8cases of ACS undergoing elective PCI(including6cases of AMI,2cases of unstable angina). Coronary angiographyshowed that32patients had two or more multi-vessel disease, eight cases ofsingle-vessel disease. Among40cases of ACS PCI perioperative mortality,22cases due to pump failure (55%),5cases of ventricular fibrillation and (or)cardiac (12.5%),6cases of cardiac rupture (15%),7cases of reinfarction, orstent thrombosis (17.5%). Among the18patients of Emergency PCI who dieddue to pump failure,5cases of extensive anterior AMI,5cases ofanteroseptal AMI,3case of inferior wall+posterior wall of the rightventricular AMI.3case of Inferior wall AMl,1case of high lateral and1case of lateral posterior wall.Conclusions:The main reason of ACS patients perioperative mortality of PCIare pump failure, cardiac rupture, reinfarction or stent thrombosis, malignantarrhythmias. PCI perioperative risk factors for death included old myocardial infarction, age over70years, cardiogenic shock, female, Renal insufficiencyat baseline>2.0mg/dl, left anterior descending artery occlusion, unused stentsand platelet glycoprotein GP Ⅱ b/Ⅲ a receptor blocker and multi-vesselcoronary artery disease, emergency room from the first balloon time (door-ball time), history of diabetes and so on.
