Objective:To study the hemodynamic change pattern of fetal middle cerebral artery(MCA) in pregnancy patients with preeclampsia at32~34weeks, estimate fetal weight by measuring biparietal diameter(BPD), femur length(FL), head circumference(HC) and abdominal circumference, and discuss the value of it in forecasting small for gestation age(FGR), evaluate SGA combined with fetal structural biology measure synthetically, in order to intervene and treat timely in clinic.Methods:96pregnants who visited gynaecology and obstetric at the First AffiliatedHospital of Shanxi Medical University from March2013to December2013were chosen, aged25to32, average age28years old,32~34weeks in gestation age, average33weeks,32normal cases,32mild preeclampsia cases,32severe preeclampsia cases.Use GE E8color Doppler ultrasonic tester to observe fetal, the probe frequency of transabdominal ultrasonography is3.5~5MHz.Test hemodynamic parameters of fetal middle cerebral artery: systolic phase/diastolic phase(SD), resistance index(RI) and pulsatility index(PI), estimate fetal weight by measuring biparietal diameter(BPD), femur length(FL), head circumference(HC) and abdominal circumference, evaluate SGA combined with fetal structural biology measure synthetically, in order to intervene and treat timely in clinic. Results:1, Normal cases’ fetal MCA: set of fetal weight and MCA average weight is2078.81g±223.67g, PI was1.54±-0.23, RI was0.80-0.06, S/D for2.200.62mm; Pregnant women with mild preeclampsia group of fetal weight and MCA: the average weight of1938.56g±249.9g, PI was1.34±-0.19, RIwas0.71-0.06, S/D for2.980.93mm; Severe preeclampsia group of fetal weight and MCA pregnant women: the average weight of1548.81kg+244.11kg, PI was1.13±-0.33, RI was0.51±-0.08, S/D for3.230.68mm;The parameters of mild preeclampsia cases’ fetal MCA are lower than the normal cases’, furthermore, the parameters gradually decreasing with the patient’s condition gradually deepened.2, Normal cases’ fetal pregnant women with mild preeclampsia group and severe preeclampsia pregnancy fetal weight and PI, RI and S/D, the difference had statistical significance. Which weight, PI, RI is normal pregnant group highest fetus, fetal lowest severe preeclampsia group; S/D is a normal pregnant women set minimum fetus, fetal highest severe preeclampsia group.With the patient’s condition gradually deepened, PI of mild preeclampsia cases,PI and S/Dof severe preeclampsia cases are significantly lower than the normal cases, andthere was statistical difference.3, Normal cases’ fetal, the pregnant women with mild preeclampsia group and severe preeclampsia women group of fetal weight and PI, RI and S/D are positively correlated.The PI, RI and S/D of severe preeclampsia cases are significantly lower than the normal cases, and there was statistical difference between these two groups (P<0.01).Conclusion: 1, The use of color Doppler ultrasound, detection of fetal middle cerebral arter y hemodynamics index value of PI, RI values, S/D value, can predict preecla mpsia patients, for clinical provides necessary reference value.2, Preeclampsia pregnant fetal middle cerebral artery blood flow dynamics for RI value, RI and S/D value and negatively correlated with fetal weight value.3, The two combined can be estimated fetal degrees in preeclampsia patients, t he diagnosis and treatment for clinicians hypertensive disorder complicating pre gnancy provides some necessary help, combined with fetal about structural biol ogy measurement, a comprehensive evaluation of SGA for clinical intervention and treatment in time.
