Objective:To evaluate the effect of pregnancy-induced hypertension(PIH)on the middle cerebral artery(MCA)in fetuses and neonates by contrast studies on the blood flow parameters of the middle cerebral artery(MCA)in fetuses and neonates between the normal control group and the experimental group.By this way,we can understand the effect of pregnancy-induced hypertension on the cerebral blood circulation of fetus and newborn.Thus providing the reference data for evaluation on preclinical monitoring target and therapeutic effect of pregnant women,fetuses and neonates.Providing the reference data for evaluating the degree of brain damage in neonates.At the same time,we can investigate the relevance on the blood flow parameters of middle cerebral artery between fetus and newborn of pregnancy-induced hypertension wemen.Methods: Randomly selecting 72 pregnant women who have builded archives and deliverd in our hospital.Those pregnant women were diagnosed with pregnancy-induced hypertension by clinical symptom performance,laboratory examination,cardiovascular system examination and physical examination.Their pregnancy time were 32 weeks full and above.There have 72 cases of subjects in experimental group(divided into severe preeclampsia(52),mild preeclampsia(11),gestational hypertension(9),group 3).Their average age were 30.4 years old.They were 32 ~ 40 weeks pregnant and have an average pregnancy week of 37.4.The control group was consisted of 60 pregnant women with normal blood pressure.Those women were 32 weeks full and above pregnant.Those normal pregnant women have an average age of 29.7 years old and were 32~40 weeks pregnant.They have an average pregnancy weeks of 38.1.All cases without suffering from other diseases.Applicating spectrum Doppler Detection Technique to detect parameters of middle cerebral artery of fetal and neonatal brain in those four groups including pregnant women who were induced hypertension or normal.The work of collecting data was finished by a fixed person who was an experienced ultrasound physician and has finished the professional training.The survey of fetus was going on when a woman was hospitalized,at third day and at seventh day after admission.The survey of newborn was enforced within a week after birth.The spectral Doppler parameters included peak systolic velocity,end-diastolic velocity,the ratio of peak systolic and diastolic blood flow velocity,the ratio of S/D,resistance index(RI)and pulsatility index(PI).Results: Using SPSS19.0 statistic software package to analysis data statistic.All measurement data were expressed in x ±s.The index of p<0.05 representsed statistically significant difference.The value of PSV and EDV of middle cerebral artery in fetus were gradually going down from normal control group,pregnancy hypertension group,mild preeclampsia group and severe preeclampsia group.There was no significant difference in the blood flow parameters of fetal middle cerebral artery(S/D?PI and RI)between gestational hypertension group and normal control group(P>0.05).There was significant difference in S/D?PI and RI between mild preeclampsia group?severe preeclampsia group and normal control group(P<0.01).The blood flow parameters of the middle cerebral arteries in the first two groups were lower than those in the normal control group.There was significant difference in S/D?PI and RI between mild preeclampsia group and severe preeclampsia group(P<0.01).The blood flow parameters of the middle cerebral artery in the previous group were higher than that in the latter group.At 3 and 7 days after treatment,the blood flow parameters of fetal middle cerebral artery(S/D,PI and RI)in severe preeclampsia group were significantly higher than those before treatment.Parameters of middle cerebral artery in newborn,The value of PSV and EDV of middle cerebral artery in fetus were gradually going down from normal control group,pregnancy hypertension group,mild preeclampsia group and severe preeclampsia group.But Edv was more pronounced than PSV changing trend.There was no significant difference in blood flow parameters of middle cerebral artery(S/D?PI and RI)between the pregnancy hypertension group and the normal control group(P> 0.05).Separately comparing the mild preeclampsia group?the severe preeclampsia group with normal control group,the blood flow parameters(S/D?PI and RI)of the first two groups were higher than the normal control group.There was significant difference in the above parameters(P<0.01).The parameters of severe preeclampsia group and mild preeclampsia group were significantly different(P <0.01),and the former parameter was lower than the latter.Conclusions: Through the detection of the blood flow parameters(S/D?PI and RI)of the newborn and fetal middle cerebral arteries in the patients with pregnancy-induced hypertension.It was found that the parameters(S/D?PI and RI)of middle cerebral arteryin the newborn and fetal whose mother was hypertensive patients during pregnancy were different from those of normal pregnant women.With the apostasis of pregnant women,the parameters(S/D?PI and RI)of middle cerebral artery change obviously.Therefore,the blood flow parameters of middle cerebral artery(MCA)in fetuses and neonates whose mother were with pregnancy-induced hypertension could be determined qualitatively.They can help to accurately determine fetal intrauterine blood oxygen status and neonatal cerebral blood flow circulation and to provide a more accurate clinical treatment. |
