Background: Lung infections, pulmonary atelectasis and other pulmonarycomplications are directly related to the recovery of general thoracic surgery patients,especially elderly patients. So what is the effective prevention of pulmonarycomplications after thoracic surgery needs to be solved. With the development ofmedical industry, the patient’s age increasing, lung protection method is also rapidlyprogressed. However, in clinical practice, although there are various of methods forthe protection of the lungs, the damages caused by prolonged intubation andduring the operationare still inevitable, including complications such as pulmonaryinfection and pulmonary atelectasis, especially for elderly patients.In recent years, with the understanding in the mechanism of general thoracicpostoperative pulmonary complications, some scholars recommend the use ofgeneral thoracic mucosolvin in postoperative pulmonary complications prevention.Domestic and international studies show that the protective effect of mucosolvinlagainst postoperative pulmonary is obvious, especially in high doses. However,neither the dosage nor length of medicine use has no unified standard.Objective:(1) To explore the different effect on pulmonary complications in patientsafter general thoracic surgery using different doses of mucosolvan;(2) To provide evidence for reducing the incidence of postoperativepulmonary complications;(3) To explore the mechanism of postoperative pulmonary complicationsafter thoracic and general thoracic surgery and the basic mechanismsof action and the meaning of prevention of pulmonarycomplications.Methods:To select60patients who was diagnosed non-small cell lung in theThoracic Surgery of The Tumor Hospital of Shanxi between2012and2013. All ofthem was operated by the same group of doctors in the Thoracic Surgery of TheTumor Hospital of Shanxi through the double lumen endotracheal intubation generalanesthesia downlink lung resection. All patients were randomly divided into three groups,20cases in high-dose group,20cases in middle dose group, and20cases inlow-dose group. In addition to the routine anti-infection and nutritional supporttreatment after operation, high-dose group were given mucosolvan about1000mg/dcontinuously; medium dose group were given mucosolvan about600mg/dcontinuously and low-dose group were given mucosolvan about300mg/dcontinuously.7days later we observed their sputum culture, sputum characteristics,clinical manifestations, chest X-ray, and so on to identify pulmonary infection,pulmonary atelectasis and other complication of lung, and make comparative study.Results:(1) There are differences among high-dose group, middle dose group andlow-dose group in the rates of postoperative pulmonary infection afterusing mucosolvan seven days. The difference was statisticallysignificant (p <0.05). The postoperative pulmonary infection rates ofpatients with high-dose group is lower than moderate-dose group andlow dose group;(2) There are differences among high-dose group, middle dose group andlow-dose group in the rates of postoperative pulmonary atelectasisafter using mucosolvan seven days. The difference was statisticallysignificant (p <0.05). The postoperative pulmonary atelectasis rates ofpatients with high-dose group is lower than moderate-dose group andlow dose group;(3) There are differences among high-dose group, middle dose group andlow-dose group in the rates of postoperative pulmonary complicationafter using mucosolvan seven days. The difference was statisticallysignificant (p <0.05). The postoperative pulmonary complication ratesof patients with high-dose group is lower than moderate-dose group andlow dose group.Conclusion:(1) The protection of postoperative pulmonary infection of patientswith high-dose group is better than moderate-dose group and lowdose group;(2) The protection of postoperative pulmonary atelectasis of patientswith high-dose group is better than moderate-dose group and lowdose group;(3) High dose of mucosolvan can effectively reduce the rate ofpulmonary complications after general thoracic surgery such as lung infections and pulmonary atelectasis.
