Clinical Study On The Relation Between Oxcarbazepine Concentration, Epilepsy Therapy Efficacy And Safety

Objective:By establishing database of epilepsy,we extract the patient’s information who treated by Oxcarbazepine(OXC), to observe and evaluat the relevance between concentration and clinical curative effect and safety of its major active metabolite,the10-hydroxy-10,11-dihydro-5H-dibenz(b,f)azepine-5-carboxamide,also named monohydroxylated deribative (MHD).And provides reasonable evidence for clinical workwith OXC.At the same time,we can improve the experience of epilepsy database management, to promoting the ability of long-term management of treatment in patients with epilepsy.Methods:Collected the information of adult outpatients and inpatients, diagnosed with epilepsy from2013-02to2014-02at the second affiliated hospital of Soochow university. Records the the basic information such as name, age, height, weight, and the dosage combination with other ADEs in epilepsy database.We choosed the samples with OXC.The serum trough steady state MHD concentration of post-dose samples was measured by high-performance liquid chromatography(HPLC).And follow-up the related adverse reaction and the clinical curative effect of three months later.At the same time analysised the relationship between concentration and dose to weight ratio(dosage/weight), and investigated the concentration and the clinical curative effect.Result:92cases of epilepsy patients were collected,and42cases of OXC therapeutic drug were analyzed.There are28cases of male,and14cases of female, age from16to77years old, mean age (37.2±17.9),height (169.0±6.7)cm,weight (65.7±11.6)kg, BMI(23.0±3.5).The total effective rate was76.47%,and the effective rate was91.3%in the effective therapeutic window groups,64.2%in the groups of below effective therapeutic window.After thex2inspection,the effective rate is better (P<0.05); A linear relationship between serum concentration and dose to weight ratio was found: C=0.275×D+6.86(R=0.402,P=0.03).8cases of adverse reaction were found, The incidence was19.0%.The incidence of OXC-induced was low, and no severe reactions occurred.Conelusions:Oxcarbazepine is efficacious and well tolerated in adults and adolescents.Detecting concentration can help us make a good individualized drug administration,understand the correlation of adverse reactions and control the seizures.
