The Prognostic Significance Of Blood Routine Parameters In Patients With Clear Cell Renal Cell Carcinoma

Objective: To evaluate the prognostic significance of neutrophil count,lymphocyte count,neutrophil-lymphocyte ratio,hemoglobin,platelet count,platelet-lymphocyte ratio in the preoperative blood routine parameters in patients with clear cell renal cell carcinoma (CCRCC).Methods:The clinical data of292CCRCC patients, firstly diagnosed and confirmed by postoperative tissue diagnosis, was analyzed retrospectively and all of them were followed-up. The292patients were divided into two groups by the values of normal limits of hemoglobin(anemic and normal group)and platelet count(thrombocytosis and normal group)from the seventh edition of "diagnostics" and Higher and lower group by the optimal cut-off values of the other four parameters selected by minimum p value approach.Kaplan-Meier, Log-Rank test and Cox proportional hazards regression model were used to analyze the data.Results:The optimal cut-off values of neutrophil count(3.47×109/L,P=0.025), neutrophil-lymphocyte ratio(1.28,P=0.042)and platelet-lymphocyte ratio(200,P=0.001)were statistically significant. Univariate analysis showed that way of onset,neutrophil count,neutrophil-lymphocyte ratio,platelet count.platelet-lymphocyte ratio,tumor necrosis, grade, T stage, tumor size and clinical stage were prognostic factors.Multivariate analysis showed that neutrophil count(P=0.032), T stage(P=0.032)and clinical stage(P=0.009)of CCRCC patients (progression free survival) were independent prognostic predictors and clinical stage(P=0.049) of CCRCC (overall survival) was also independent prognostic factor.Conclusion:Higher preoperative peripheral blood neutrophil count is an independent poor prognostic factors for CCRCC patients.Pathological T stage and clinical stage of CCRCC patients were also independent prognostic predictors.
