Background: With the development of the social economy and the improvement ofliving standards in recent years,especially in the worldwide incidence of diabetes isincreasing rapidly in developing countries,of which more than90%of patients withtype2diabetes.The number of patients with type2diabetes in China ranked the topthree worldwide. Because type2diabetes is often accompanied by severecardiovascular complications, thus giving our health care system and the enormoussocial and economic burden on health care. Type2diabetes which is a chronic insulinresistance induced by high glucose is the main feature of metabolic diseases. Insulinresistance and impaired insulin secretion is the main mechanism of type2diabetes,but the current of this mechanism of metabolic abnormalities remains unclear. Inrecent years, more and more research data and experimental evidence abroad haveshown that chronic subclinical inflammation may be involved in the pathogenesis ofinsulin resistance and type2diabetes. May be derived macrophages, fat cells andendothelial cells of long-term excessive secretion of inflammatory cytokines causepancreatic β cell function impaired and insulin resistance (IR).Through the analysisof pre-diabetes and diabetes clinical features and biochemical research we may findthe related factors with new-onset type2diabetes and early onset of type2diabetes,We can clarify chronic inflammation in the progression of type2diabetes. In order tofacilitate the detection, monitoring and easy method of disease prognosis, achieve thepurpose of prevention and early intervention to reduce the incidence of acutecardiovascular events. Purpose: By studying the serum levels of hs-CRP, MIF, RANTES expression levelsin subjects With varying glucose metabolism status, As well as their role in the earlyindependent predictor of type2diabetes and type2diabetes, We can further studythe relationship between the inflammation and impaired glucose regulation, type2diabetes. And we can also know the chronic low-grade inflammation is involved inthe development of type2diabetes.Method: Subjects were selected from June2013to December2013in our hospitalmedical center medical staff, outpatient and inpatient endocrinology, subjects weredivided into three groups according to an oral glucose tolerance test (OGTT):(i)30healthy volunteers with normal glucose tolerance(NGT).(16males and14females)FPG<5.6mmol/L and2hPG<7.0mmol/L;(ii)30patients with impaired fastingglucose(IFG) or with impaired glucose tolerance(IGT).(17males and13females).5.6mmol/L≤FBG<7.0mmol/L or7.8mmol/L≤P2hBG<l1.1mmol/L,;(iii)30patients with newly diagnosed,complication-free type2diabetes.(17males and13females). FBG>7.0mmol or P2hBG>l1.1mmol/L. Subjects who were fastedovernight after10h underwent OGTT, measured FBG, fasting insulin (FINS), P2hBGand2h insulin and homeostasis model assessment insulin resistance index(HOMA-IR); simultaneous determination of glycated hemoglobin (HbA1c),triglyceride (TG), total cholesterol (TC), inflammatory cytokines high-sensitivityC-reactive protein, RANTES, MIF expression were measured by ELISA detection.Results:1.general analysis of clinical data and biochemical indicators compareThe results showed that age,gender,height, weight, diastolic blood pressure, totalcholesterol, creatinine, urea nitrogen, ALT, PLT was no significant difference amonggroups;Compared with NGT group, the IGR and T2DM has significantly higherlevels of DBP,HDL,WBC,HB,2hFINS,HOMA-IR;the IGR and T2DM hassignificantly higher levels of hs-CRP, RANTES, MIF compared with NGT group.2Correlation analysisThe serum levels of hs-CRP correlated with BMI,FINS and HOMA-IR.The serum levels of MIF correlated with2hPG,HbA1c,FINS and HOMA-IR.The serum levelsof RANTES correlated with FINS,DBP,LDL and BUN. Hs-CRP significantlycorrelated with RANTES and MIF.3. Multivariate regression analysisTake hs-CRP,MIF,RANTE as independent variable,IR as dependent variable, Doingmultivariate regression analysis,affecting factors of IR were hs-CRP,MIF. hs-CRPplays the biggest role in both IGR and T2DM.
