Objective: To investigate the effects of tranexamic acid(TA) onperioperative blood loss in the primary cementless total hipreplacement(THR).Method:The patients underwent the primary cementless THRfrom the June2012to July2013were randomly divided into2parts in the prospectivestudy.We do not use the tranexamic acid in A group;and use the tranexamic acid in Bgroup preoperatively and postoperatively.The two groups were given the oralanticoagulation(Rivaroxaban10mg/d)after the surgury. The intraoperative bloodloss,the amount of postoperative drainage,the amount of postoperative3h、6h、9h、12h drainage,the total blood loss,the number and the ratio of the patients receivedblood transfusion,the ratio of VTE and the average hospitalization days afteroperation were recorded and compared between the2groups.Hemoglobin,hematocrit,the amount of the blood platelet,prothrombintime,activated partial thromboplastin time,D-Dimer and fibrinogen degradationproducts were also recorded and compared preoperatively and postoperatively.Allpatients checked the double limb vein ultrasonography in the14th day after theoperation.Results:The blood loss was (302.22±52.86)ml in Group A and(244.00±103.99)ml in Group B during operation,there was no significant differencebetween the two groups(p>0.05).The postoperative drainage volume was(466.11±63.17)ml in Group A and (273.44±163.01)ml in Group B,and the total bloodloss was (726.72±503.47)ml in Group A and (353.47±167.69)ml in Group B,therewere significant differences between the two groups(p<0.05).There were24patientsin group A and5patients in group B accepted transfusion,the ratios are53%and11%(p<0.05),and the average amount of blood transfusion was (750±430)mland(360±89)ml(p<0.05).The patients of the two groups both showed no venous thrombosis of lower extremity with the inspection results of the double limb veinultrasonography.The average hospitalization days of the two group after operationwere (7.98±3.11)days and (6.53±1.38)days(p<0.05).Conclusion:TA reduce thepostoperative drainage volume and the total blood loss in the primary cementlessTHR,and it also decrease the blood transfusion ratio and the average hospitalizationdays after operation without increasing the happening of the venous thrombosis oflower extremity.
